Description/Job Summary


The Team Lead provides supervision and training to Case Managers who provide services to Community Support Services (CSS) participants who have been diagnosed with serious and persistent mental illness. This includes field supervision of Case Managers, the provision of direct clinical services to participants, the provision of clinical direction for services planning, participation in team and individual case management, participation in CQI and Clinical Issues activities, and the collection of outcomes measurement data. Under the direction of the Regional Director or Assistant Regional Director, the Team Lead works in collaboration with case managers and other team members to ensure a high-quality and appropriate level of care. While adhering to specific procedures and policies, the Team Lead will assist to empower the adults with serious and persistent mental illness by directing services toward achieving the goals necessary for success and satisfaction in the community. The Team Lead will also support the Regional leaders in implementing systems for Corporate and Divisional goals. A commitment to the RHD values should be demonstrated as job duties are performed.

Responsibilities/Duties


* Work closely with Case Managers to ensure that they are responsive to consumer needs and effectively provide the services that they most benefit from.
* Support Case Managers in daily problem solving regarding challenges that arise on a daily basis by working directly with consumers in crisis.
*Maintain a caseload of Case Management consumers.
* Review rehabilitation plans and Case Manager interventions at a regular interval, to assess for progress and to recommend adjustments to interventions provided or adjustments to the rehabilitation plan as appropriate.
* Utilize a psychiatric rehabilitation approach, a demonstrated understanding of Community Support Services and the recovery model in direct work with consumers and in guiding and training Case Managers.
* Engage participant in a trusting and supportive relationship, providing emotional support within professional boundaries. Lead Case Managers in doing the same.
* Provide on-call coverage which may include phone work and possibly in-person contact.
*Assume an active role as liaison with family members, community agencies, advocacy groups, and other invested individuals and groups.
* Provide crisis intervention services in the homes of consumers as needed.


*Guide Case Managers to receive and orient new consumers to their homes and communities.


*Assist with search for available housing for consumers in need.


* Inform Assistant Regional Director and Regional Director regarding pressing clinical issues effecting clinical program consumers.


*Ensure that services provided are consistent with NJ DMHAS CSS regulations.


Required Experience


Experience with programs designed to support individuals with mental illness, addictive disorders or other life challenges; clinical experience required.
* Excellent communication, written and time management skills


* Extensive knowledge of healthcare, community treatment, support services and resources

*Ability to work effectively with diverse team and remain highly organized and self-motivated


Required Education

* Master’s degree in human services field.

Program Summary

The Resources for Human Development (RHD) SALT Passaic Supportive Housing Program is a Community Support Services (CSS) provider and provides intensive case management supports in the homes of individuals with psychiatric illnesses.  SALT Passaic links consumers of mental health services to safe, affordable housing and individually assists each consumer to meet his/her chosen goals. Overall, the goal of the program is to support individuals to maintain community residence, mental and physical health, and to achieve any other identified individualized goals.  Services are provided in the community and link individuals to any available community resources that can assist in their support.
