Description/Job Summary

The Clinician provides clinical services to Community Support Services (CSS) participants who have been diagnosed with serious and persistent mental illness. This includes creation of Individualized Rehabilitation Plans, completion of initial and follow-up Comprehensive Rehabilitation Needs Assessments, the provision of direct clinical services to participants, the provision of clinical feedback for service provision, participation in team and individual case management meetings as needed, participation in CQI and Clinical Issues activities, and the collection of outcomes measurement data. Under the direction of the Regional Director, the Clinician works in collaboration with Case Managers and other team members to ensure a high-quality and appropriate level of care. While adhering to specific procedures and policies, the Clinician will assist to empower the adults with serious and persistent mental illness by providing and assisting Case Managers to provide services that will assist consumers to meet the goals necessary for success and satisfaction in the community. A commitment to the RHD values should be demonstrated as job duties are performed.

Responsibilities/Duties

  • Conduct screening and assessment in accordance with the use of validated

           screening tools to screen and assess for depression, trauma, substance use,

           and risk/needs associated with criminal risk.

  • Develop Individualized Recovery Plans and reviews in collaboration with

the consumer, their Case Manager, and any other consumer-requested participants.

  • Maintain a reasonable caseload of Case Management consumers.
  • Assist with the assessment of newly referred consumers.
  • Document all treatment contacts as required by applicable regulations.
  • Demonstrate the ability to provide treatment that is recovery-oriented, supports evidence-based practice and can clearly establish the status of the client in treatment
  • Provide individual psychotherapy utilizing approaches such as CBT, DBT, Cognitive Therapy, and Motivational Interviewing.
  • Seek opportunities to provide formal and informal education to natural supports
  • Provide education to team members regarding assessment and interventions
  • Meet monthly required levels of treatment contacts
  • Provide 24-hour emergency and crisis intervention response on rotating, on-call basis
  • Complete assessments, progress notes, recovery plans and significant data in required time periods
  • Provide culturally competent services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability,  handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex
  • Maintain confidentiality and adhere to HIPAA
  • Perform other duties as required by the Regional Director.

Required Experience

  • Experience understanding various models and theories pertaining to treatment of serious mental illness and risk management
  • Experience with programs designed to support individuals with mental illness, addictive disorders or other life challenges.
  • Excellent communication, written and time management skills

Required Education

  • Master’s Degree in Human Services related field.

Required Qualifications

  • Master’s Degree in Human Services related field.
  • Professional license in related field.
  • Valid driver’s license

  • Criminal record clearances, as dictated by the assigned programs


Program Summary

 The Resources for Human Development (RHD) SALT Passaic Supportive Housing Program is a Community Support Services (CSS) provider and provides intensive case management supports in the homes of individuals with psychiatric illnesses.  SALT Passaic links consumers of mental health services to safe, affordable housing and individually assists each consumer to meet his/her chosen goals. Overall, the goal of the program is to support individuals to maintain community residence, mental and physical health, and to achieve any other identified individualized goals.  Services are provided in the community and link individuals to any available community resources that can assist in their support.
