Description/Job Summary

The Quality Assurance Analyst is responsible for developing systems of quality measurement, including but not limited to, trending data, creating reports, facilitating meetings, training, and overall implementation of the division’s quality assurance plan.  The Quality Assurance Analyst works with Unit Directors and the Quality Assurance Manager to coordinate quality services and provides person-centered and trauma-informed supports to the people we serve.

Responsibilities/Duties

  • Ensure the development of ISP goals in conjunction with each team to address medical, health and safety, psychiatric, behavioral and social needs for individuals served.
  • Review systemic compliance with all licensing and accreditation standards governing services.
  • Complete site/environmental inspections, data tracking and reporting trends related to: health and safety, behavior, customer satisfaction, staff retention, turn-over, licensure and certification follow up.
  • Conduct audits to assess readiness for licensing and provider monitoring.  Complete quarterly reviews of each residential home, day program (if applicable), and administrative site.
  • Routinely review consumer files to ensure that all required documentation and forms are present and up to date.
  • Review employee training and personnel files on a regular basis to ensure that they are up to date with training and background checks.
  • Provide clear and concise reports on findings related to Quality Assurance activities and report findings to Quality Assurance Manager and Quality Assurance Directors for execution and follow up.
  • Retain comprehensive, working knowledge of the policy and procedures and work with Regional Directors and program leaders on proposed revisions as needed.
  • Facilitate and provide training as needed. Work with the divisional leaders on implementing and training staff on EHR system and related technologies.
  • Support and participate in the ongoing assessment of the quality and effectiveness of program throughout the division.
  • Address issues of concern with appropriate Unit Director and assist with the development of strategies to address the areas of concern.
  • Remain current in training methods and best practice related to quality assurance, person centered thinking, assessments and key performance indicators.
  • All other related duties assigned.

Required Experience

A minimum of three years in a human services field.

Required Education

Bachelor’s Degree in a Human Services field, including but not limited to, sociology, special education, rehabilitation, nursing, counseling, psychology.  Master’s Degree in related field is highly preferred.

Required Qualifications

  • Extensive knowledge of Microsoft Office - charts, grafts, and report writing.
  • Training and background in person centered planning, assessment and supported employment is a plus.
  • Superior organizational, clinical, interpersonal, and management skills along with an excellent administrative, time management and computer skills.
  • Knowledge of Quality Assurance.
  • Must have valid driver's license.
