This is a professional level position that will have the ability to work from home. However, in-state travel is required. The overall responsibility of this position will be administering the CHOICES waiver and CTS program, and coordinating services for
adults and children who have an intellectual or developmental disability. This includes, but is not limited to, coordinating services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational,
legal, and advocacy needs. Case Managers will provide support to individuals to actively engage in and direct the person centered plan development process.
Case Manager will lead person centered planning efforts, focusing on what is important to and important for those on their case load, and designing the supports that will be most effective in achieving
a balance between the two. Case Managers will advocate for those on their caseload, assisting with decision making about the supports and services they want and need, and facilitating the provider selection process.
Compensation includes a competitive hourly rate, mileage reimbursement, and initial cost of home office set up.
Essential Duties and Functions
- Facilitate development of ISP
- Facilitate pre-meeting
- Work with each participant to invite team members for in=person meeting
- Conduct and coordinate assessment completion
- Write the Standardized ISP
- Assure implementation of ISP
- Assume lead over Person Centered Planning efforts, including utilization of discovery and PCT tools in accordance with HCBS regulations
- Facilitate annual ISP
- Facilitate team meetings as changes are needed or as required.
- Ongoing monitoring of ISP
- Complete all components of ISP monitoring quarterly, including face to face contact
Ensure follow up from Community Support Provider
- Submit monitoring to the support team
- Conduct monitoring in multiple settings/locations for a participant where supports are being provided
- Critical Incident Reports
- Review Community Support Provider internal incident reports
- Identify Critical Incident Reports and report to Division of Developmental Disabilities
- Investigate Abuse, Neglect, and Exploitation reports when related to allegations involving RHD Case Management
- Reporting responsibilities to families and guardians
- Follow up on Critical Incident Reports and facilitate changes to plan if needed (preventative measures)
- Ensure follow up by RHD and Community Support Provider
- Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC)
- Review approval prior to implementation and annually thereafter
- Participate in HRC/BIC meetings
- Assist as needed with completing documentation, and submitting to HRC/BIC
- Review recommendations from HRC/BIC and ensure are included in plan
- Assist in obtaining consents for psychotropic medications when time permits
- Identify emergency restrictions, and document and report to HRC/BIC and team within 24 hours
- Focus on Personal Outcome measures
- Evaluate the needs of those on caseload in order to remain successful in the community
- Link those on caseload to programs and supports that are needed and requested to meet their needs
- Maintain caseload of individuals with intellectual or developmental disabilities
- Complete required documentation of services, including input of data, reports, and plans into Therap
Qualified candidates will possess a bachelor’s degree or equivalent in human services, social work, sociology, or psychology, and 2 years of experience working in a similar role. Work experience may be substituted for education. Background and experience
in facilitating Person Centered Planning is highly preferred. Have the ability to establish a HIPAA compliant home office.
