The Resource Coordinator provides assistance to program participants in obtaining a wide range of services and support through various government and non-profit agencies including, healthcare, housing, counseling services, job placement, recovery and support groups, and other support programs identified with the support of medical, clinical and direct support staff at the program. The Resource Coordinator will collaborate with the Unit Director, therapists, and the nursing in providing a seamless and efficient service delivery to the participants served.
Orient the new participant to the program, consent for treatment, release of information, rights and responsibilities.In collaboration with medical and clinical team identify clinical, social, medical and long term needs newly admitted resident.
As needed complete DPW & Social Security Application with new residents.
Coordinate and participate in reviews with internal clinical team and external partners including funder, government agencies and family to review individual’s progress.
Documentation of treatment goals/quarterly reviews, discussions, and progress.
Complete Discharge summary for new intake and update for every resident every six months.
Coordinate outreach services when an individual is hospitalized or in the community
Review clinical charts and perform a monthly clinical chart audit within the first week of a new month.
Maintain clinical charts according to state and local regulations.
Complete required paperwork in compliance with agency regulations including submission of resident event forms and tracking forms.
Perform on-call responsibilities as part of the administrative team
Accompanies residents to appointments and/or recreational activities on public transportation at the direction of program director or on-duty nurse
Coordinate appointments and interface with community agencies and resources
Experience within Drug and Alcohol/Criminal Justice population
Experience with Microsoft Office
Valid Driver’s license required
ACT 33/34 clearances
FBI clearance, as needed
