Description/Job Summary

Position Summary

RHD Community Choices is a program that provides trauma-informed psychosocial rehabilitation in a residential setting with staff coverage up to and including 24/7 supervision, based on the participants’ needs. The Lead Direct Services Professional (LDSP) is directly responsible for creating an environment that reflects the RHD mission while simultaneously upholding our values. The LDSP assists the management team with managing the sites and clients at those sites. LDSPs assists participants, as needed, in living as independently in the community as possible. Services include assistance in self-care skills and health care (including medication management, medical appointments, programs, housekeeping, nutritional needs, mobility, money management, interpersonal skills, vocational/education pursuits, recreation, time management and community participation). This position will provide direct care in on or more of our Chester County locations.

Responsibilities

Maintain a Safe and Open Environment

  • Get to know the residents and the property. Engaging with, building trust and gaining a good rapport is the key to a successful professional relationship.
  • Intervene in crisis situation and document all critical incidents (Incident Report must be completed within 24 hours of the incident).
  • Conduct safety checks on participants “in and out of the facility” to ensure safety and appropriate behavior.
  • Participate in shift changes and fire drills along with any other evacuation or emergency incidents to ensure the safety of our residents and staff.
  • Ensure that the kitchen, bathrooms and all common areas are clean and disinfected.
  • Other duties assigned by direct supervisor as needed.


Health and Wellbeing

  • Provide Medication Monitoring (monitor residents taking medication, counting narcotic medication and reviewing medication books for accuracy). Let the residents know when their medications are low and if they have refills. Assist as needed with reordering medications, making doctor appointments, understanding what the medications are for and their possible side effects.
  • Escort residents to appointments as needed utilizing the company van.
  • Heat/Serve meals to consumers at scheduled times. Label, date, and put away leftovers, wash dishes, wipe down counters and damp mop the floors.
  • Intervene in disputes before they become crises.
  • Provide supportive interventions to encourage resident’s effort to accomplish tasks and be available to meet with residents individually as needed.
  • Encourage residents to adhere to their recovery plans, be active in in-house activities as well as outside activities.
  • Model healthy behaviors, always treating people with respect. Strive to implement the RHD Values in the workplace. Follow all policies and procedures from RHD and Community Choices.
  • Identify areas for improvement throughout the program and in staff development.
  • Attend ALL mandatory staff meetings, trainings and supervisions.
  • Maintain appropriate relationship and environmental boundaries at all times.


Documentation

  • Answer phone and document messages, activities, observations and other information in logbook, related forms, shift summaries, and other documentation
  • Utilize Microsoft Office, Lawson System, DAP Notes, Communication Notes, and reporting of required submissions of necessary data in a timely fashion.
  • Maintain resident files and records
  • Read the communication log, DAP Notes and emails in the beginning of your shift
  • Complete all trainings

Required Experience


    2 or more years Mental Health Experience required2 or more years of managerial experience preferred 

Required Education

High School diploma or GED

Required Qualifications

  • Driver's License
  • Clear criminal background and other background checks in accordance with state requirements
  • Ability to pass all required trainings
  • Ability to convey, understand, and follow spoken and written instructions in English
