Direct Support Professional
 
RHD's Community Choices 20 is seeking caring and motivated staff to assist individuals with mental health and/or physical disabilities with learning job skills and/or activities of daily living in our residential support programs. This position will provide staffing in our West Chester, Downingtown and Coatesville locations. Also, must be motivated to work in a team environment. Competitive pay and benefits package.
 
Duties and responsibilities

  • Engage the participant according to established supervision levels and focus on targeted areas of recovery.
  • Assist in the process of identifying, developing and implementing the Individual Recovery Service/Behavioral Plans.
  • Implement the goals from the participant’s recovery service plan and/or from therapist/consultants.
  • Assist the participants with day to day activities and implement social skills.
  • Support and Encourage participants in movement towards greater independence.
  • Support participants to successfully integrate into the community, such as, teaching them transportation skills, connecting them to community activities and resources, and helping them identify gainful employment.
  • Complete and submit all program documentation /data collection forms.
  • Communicate with staff at shift change about the status of the participant.
  • Prepare, assist with preparing, or supervise menu planning and meal preparation as needed.
  • Assist the participants by supervising and assisting with, as needed, the cleaning of the home and identify needs for repairs.
  • Assist the participants, in the maintenance or to maintain the grounds of the home and identify needs for repairs.
  • Assist participant with making and attending medical, psychiatric and all other appointments.
  • Provide access to approve recreational and leisure activities within the home and in the community.
  • Assist/teach self-medication administration for participants as prescribed (after completion of approved training-Quick Mar).
  • Attend all scheduled trainings, supervisions and staff meetings
  • Act in accordance with Value’s, Beliefs of RHD and to follow all policies and procedures of the Program.
  • Document and notify the Program Manager (or any other member of the management team) for all unusual incidents in the mandated timelines.
  • Maintain appropriate relationship and environmental boundaries at all times.

Salary: $13.50 to $15.00 /hour

Required Experience

 Experience assisting individuals with mental health and/or physical disabilities is strongly recommended.

Required Education

High School diploma or GED

Required Qualifications

  • Driver's License
  • Clear criminal background and other background checks in accordance with state requirements
  • Ability to pass and complete all required trainings
  • Ability to convey, understand, and follow spoken and written instructions in English
  • Experience preferred
