Summary: RHD impACT Team Clinician works with team members in collaboration with RHD impACT participants to provide assertive, clinically sound, mental health services. The interventions of the community team are designed to respond to the strengths and needs of adult citizens of Westmoreland County, diagnosed with a serious and persistent mental illness and having met the criteria for participation in the RHD impACT team.
Job Responsibilities:
Assessment of individuals referred for therapy and the provision of clinical direction for services planning and treatment goals
Conducting individual therapy
Utilizing member in vivo experiences to maximize learning and use of skills and developed insights
Participation in team and individual case management activities
Facilitation of treatment team meetings
Discharge planning
Participation in Performance Improvement and Clinical Issues activities
Collection of outcomes measurement data.
Under the direction of the Program Director, and in collaboration with the team and team leaders, the team clinician will work positively with other staff, local professionals and providers and community members. While adhering to specific procedures and policies, the team clinician will assist to empower the adults with serious and persistent mental illness to direct services toward achieving the goals necessary for success and satisfaction in the community.
Qualifications:
The RHD impACT Team Clinician must have a Master’s degree in psychology, social work, or counseling and two years experience working, preferably in a community based setting, with persons with severe mental illness.
The RHD impACT Team Clinician must be understanding of and interested in working with persons with severe mental illness in the community and home environment.
The Team Clinician is expected to have engagement skills and competence in mental health, as well as a desire to expand their knowledge base.
The Team Clinician must be flexible and able to respond appropriately and effectively in a crisis situation, providing leadership in crisis resolution activities.
ability and experience to make independent decisions is a critical component of this position.
The Team Clinician must demonstrate strong written and verbal communication skills.
The Team Clinician must also have a valid driver’s license
