POSITION OVERVIEW: The Program Specialist position holds the responsibility of supporting health, safety, and growth of the individuals in the program; including medical requirements, programmatic concerns, incident reporting and individual finances. Create an environment that empowers consumers with skills, resources and supportive services to enhance self-esteem, self-awareness, self-determination and self-respect and is conducive to the consumers personal safety, growth and development
Schedule, chair and document ISP meetings and consumer reviews along with SC
Develop and implement annual ISP on all consumers, along with SC’s and other members of the consumer’s team. Ensure all ISP’s are accurate and person-centered
Attend all team meetings pertaining to consumers to ensure proper supports are in place
Schedule and co-chair Peer Review meetings along with Unit Director
Develop and implement annual assessments on all consumers, participate in all SIS Assessments and complete all other assessments and surveys as required
Initiate innovative strategies and creative programming for consumers
Work with the Residential Supervisors and Residential Advisors to identify the needs of the consumers and prioritize skills to be developed
Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually
Create, review and implement individual goals and data sheets based on assessment and ISP to monitor consumer progress
Assist consumers in determining employment readiness and connecting with a job coach
Complete intake assessment for all new consumers along with their available family members and Unit Director
Organize, maintain and update consumer files in accordance with the DPW 6400 regulations
Monitor program records at all sites regularly and during pre-licensing
Demonstrate a strong working knowledge of DPW 6400 regulations
Demonstrate good communication, organizational, leadership and time management skills
Demonstrate the ability to work independently with a minimum of supervision
Possess a valid PA driver’s license
Demonstrate good computer skills with knowledge of Microsoft Word, Works and Excel
Possess a Bachelors Degree from an accredited college or university with a minimum of two years experience working with individuals with mental retardation or Possess an Associates Degree with a minimum of 60 credits from an accredited college or university
Four years experience working with individuals with mental retardation.
Supervised community-based residential program designed to provide a range of established and innovative rehabilitation strategies for adults with developmental disabilities and problematic sexual behaviors who are in need of clinical/forensic treatment. The program focuses on community and personal safety, accountability, competence, least restrictive alternative, normalization, due process and control.
