Beyond Boundaries provides community based living arrangements for individuals who have both intellectual/developmental disabilities (I/DD) and mental health challenges. All residents are encouraged to function as independently as possible by making decisions
and choices regarding activities of daily living and community participation. Beyond Boundaries goal is to create an environment that fosters personal growth and creativity.
Has at least 1 year experience in Behavioral Health or related field
Has completed all basic skill training and IDD Orientation
Has some knowledge of the RHD Corporate/Beyond Boundaries operating policies
Possess a valid Driver’s License
Good verbal and written communication
Demonstrates effective interpersonal skills with co-workers, consumers and family members
Has skill set in order to follow specific routines, including consumer ISP, Behavior Plan and other behavioral mechanics
JOB DUTIES AND RESPONSIBILITIES:
Maintain and have an understanding of the PA Chapter 6400 and Chapter 51 Regulations
Will provide direct care services during and complete assigned shift at an assigned site (i.e. 8-4, 4-12, 12-8). All shifts are “awake”
As a direct care employee you are an essential employee and may be asked to work additional shifts in the event of an emergency.
Assist consumers to create a house menu and shopping list
Complete monthly food shopping and purchase house supplies, as assigned
Complete all shift assignments and tasks as assigned by Site Supervisor/House Manager
Complete all site documentation (Communication Log, Medication Log, Supply Inventory, Incident Reports, Client Assessments, ISP Data, Travel Log, etc.)
Conduct Fire Drills and complete documentation as prescribed by the 6400 Regulations
Implement and document progress in all ISP and/or behavioral plans
Instruct and assist consumers with daily life skill development
Transport and/or assist consumer to and from appointments, activities and visitations
Administer consumer medication as prescribed by the doctor and complete documentation
Provide consumers with skill instruction in areas such as academics, socialization, utilizing community resources, vocational, social and interpersonal development etc.
Report needed repairs, upkeep and conditions of the physical site to the Site Supervisor/House Manager
Log into Lawson daily and submit time worked daily
Attend, participate and maintain 24 hours of yearly training, along with a completed physical examination (every 2 years) as well as all mandatory trainings per the Unit
Maintain open and effective lines of communication with co-workers to ensure continuity of care and consistency of site operations/ISP’s
Interface with Support Coordinators, Behavior Specialists, family and community resource systems in ways that enhance the reputation of RHD
Other duties as assigned
