Haldeman-Grant is a residential program that provides the services needed for an individual to maintain independence in the community around them. Haldeman-Grant support’s individuals who are Aging In Place and require support, medically, mentally, emotionally,
and socially. Haldeman-Grant fosters a nurturing atmosphere of trust, support, independence, and safety. It is a program that facilitates growth by building trust between the staff and the program participants. Support services, at Haldeman-Grant, are person-centered
and bi-lateral with an equal exchange between the participant and staff. The focus remains wellness and recovery.
Haldeman-Grant is seeking on-call, part-time and full-time Life Skills Professionals.
Essential Duties and Responsibilities
Serve as a member of Haldeman~Grant to provide support services to program participants living independently in the community.
Responsible for adhering to RHD and Haldmen~Grant’s values and beliefs and the principles of mental health recovery at all times
Abide by all policies and procedures, and perform all duties as outlined by the State of Pennsylanvia, Resources for Human Development, and Haldeman~Grant.
Maintain appropriate therapeutic boundaries while creating a supportive and professional environment at all times.
Maintain confidentiality of program participant at all times.
Support and supervise the participants according to “where they are”.
Responsible for working collaboratively with Clinical Manager and program participant to identify needs, services, and desirables.
Responsible for the development of Recovery Support Plans Goals with assigned participants at admission and at least 60 days thereafter, and supporting participants with RSP Goals daily
Discuss goal plans with the individual, breaking down the life skills to be learned into simple, measureable, and manageable short terms goals, which can be taught, modeled and practiced.
Responsible for coordinating and facilitating one monthly psychosocial group activity with program participants.
Become aware of the program participant’s strengths, and differing skills and abilities to determine the best approach and learning style that can help increase skill development.
Support/assist the program participant with the development and maintenance of necessary daily life skills to live independently within their community, i.e., cooking, shopping, household chores, seeking employment and education opportunities, medication
management, interpersonal relations, utilizing community resources, vocational, budgeting, banking, accessing transportation, meal preparation, etc.
Model appropriate social skills to help the program participant develop positive relationships, deal with interpersonal conflict and successfully participate in social, recreational and cultural activities
Responsible for providing crisis intervention when necessary.
Communicate with staff, comprehensively, at shift change about the status of all program participants
Complete weekly contact notes for assigned program participant/s in which reflect progress made towards identified recovery goals.
Monitor participant medication as prescribed (after completion of Medication Administration
Training), and complete all corresponding documentation
Immediately communicate unusual incidents, accidents, and suspected abuse or neglect to Program Manager.
Report any issues pertaining to the maintenance of the site or site vehicle in a timely fashion
Operate agency vehicles in accordance with established policy and procedures
Record time worked on a daily basis, and submit a completed timesheet on a weekly basis
Provide emergency medical procedures as needed, including Basic First Aid, and CPR (after
completion of CPR Training)
Responsible for attending supervision, clinical meetings, quarterly staff meetings, and mandatory trainings to remain current with best practices
All other duties as identified by the Program Manager and Program Director
Possess high school diploma/GED plus 2 years of experience in the social service field. CPRP, CPS, Wrap trained, a plus.
Excellent Communication skills (written and oral) including interpersonal skills
Ability to speak effectively before groups of individuals and communicate professionally with outside agencies.
Ability to manage multiple tasks or projects
Demonstrate good problem solving and conflict resolution skills
Ability to carry out duties without direct supervision
Demonstrate skills using Microsoft Office software and database management; ability to use the Internet as a resource and complete necessary documentation.
Ability to engage employees and participants in effective, therapeutic and recovery oriented interactions
Ability to support and implement RHD and Haldeman~Grant values and beliefs.
Possess a PA valid driver's license
Be 21 years of age or older
