Salary Range: $22.00-$26.00/hour
Supervisor – Assistant Unit Director
Assist person in recovery (PIR) with applying for, obtaining, and maintaining all entitlements (i.e. DPW, SSI, Managed Care)
Coordinate appointments and interface with community agencies and resources Support recovery goal attainment through linkage to community resources and creating opportunities for community integration
Complete all state mandated paperwork, i.e., Service Documents, Registration Forms
Support PIR gaining access to services with aggressive and creative attempts to help obtain resources that are needed. This includes a strong role as an advocate
Collaborate on Personal Recovery Plans which emphasize the PIR’s strength and needs, etc.
Monitor service delivery by maintaining regular contact with PIR and service providers/programs involved with the PIR.
Monitor PIR on a regular basis to note any changes that may impact recovery plan
Participate in Program milieu Attend and/or facilitate educational groups and meetings (Healthy Living Group)
Actively support residents in understanding medication management and medication assisted therapy
Medication and Health Management
Complete physicals within 7 days of admission Ensure PIRs prescriptions are current and assist PIR in obtaining prescriptions as needed
Coordinate medications and PIR health needs with program Psychiatrist Log new medication in medication log book Daily management of Medication Log Book to ensure all agency and licensing body requirements are met
Monitor the PIR’s self-administration of all medication and log in medication log as assigned
Support health needs of PIR Assist PIRs in scheduling medical appointments and follow-up on medical recommendations.
Collect and document collection of PIRs’ urine samples as assigned Program
Document educational groups and any other significant resident contact. Participate in Case Consultation Mtgs., Staff Mtgs. and supervision as assigned.
Read log daily and document in log at the end of each shift. Attend training as required by RHD and licensing bodies As assigned by Director to insure continuity of care. Job Requirements/Minimum Qualifications:
Current licensure as RN or LPN Knowledge of and experience working with homelessness, addiction, residential facilities, mental illnesses recovery preferred
Experience with Criminal Justice population. preferred Valid Driver’s license required ACT 33/34 clearances FBI clearance, as needed All trainings required by licensure
