Description/Job Summary

Position Summary
 
The Life Skills Coach is a direct service position that provides support, engagement and encouragement to persons involved in RHD CORE programs.  The Life Skills Coach supports persons involved in the program with developing routines, life skills and are available for support 24 hours per day, 7 days per week.
 
Essential Duties and Functions
  1. Support PIRs in maintaining the physical plant by monitoring and supporting completion of community jobs.
  2. Support PIRs in completing their community jobs.
  3. Maintain the building repair log.
  4. Support PIRs in maintaining daily schedule
  5. Support, assist, and monitor meal preparation.
  6. Remain awake and alert during all shifts.
  7. Assist and support cook in meal planning, food inventory, the creation of  a grocery shopping list,  and grocery shopping as assigned.
  8. Support implementation of Nutrition Program.
  9. Support, monitor and assist at mealtime.
  10. Log new medication into the program eMar as needed.
  11. Monitor the PIR’s self-administration of all medication as needed.
  12. Assist PIRs in scheduling medical appointments and follow-up on medical recommendations as needed.
  13. Collect and document collection of PIRs’ urine samples as assigned.
  14. Assist PIRs in conflict resolution and crisis prevention/intervention.
  15. Participate in Program milieu.
  16. Attend and/or facilitate educational groups and meetings (Life Skills, Daily living skills, Community Meetings) as assigned.
  17. Teach PIRs safe food preparation and storage
  18. Co-facilitate weekly Community Decision Making Group and support PIRs in menu development.
  19. Actively support PIRs in facilitating socialization and recreation activities.
  20. Participate in Case Consultation Mtgs., staff mtgs. and supervision as assigned.
  21. Read log daily and document in log when the shift begins and at the end of the shift.
  22. Escort PIRs to appointments and shopping as assigned
  23. Complete all paperwork as required by agency and licensing bodies.
  24. Provide emergency coverage for next shift.
  25. Attend training as required by RHD and licensing bodies.
  26. As assigned by Director to insure continuity of care.
Qualifications
  1. Valid driver’s license
  2. ACT 33/34 clearances
  3. FBI clearance, as needed
  4. High School Graduate or G.E.D
  5. Personal Recovery/Life Experience accepted, 3 or more years preferred
