Description/Job Summary

The Renaissance program is an adult (co-ed) residence supporting individuals with co-occurring/dually diagnosed substance abuse and mental health issue. The Medical Coordinator will work with clients seeking Co-Occurring treatment services providing supportive medical resources in a goal-directed, client-centered approach; Medical Coordinator also works as part of the interdisciplinary team to enable residents to improve the quality of life and ability to function more successfully in the community.
 
Job Type: Full-Time (40 hours per week)
 
Primary Duties and Responsibilities
  1. Monitor each service recipient’s medical and psychotropic medications to ensure that all daily medication is taken, necessary follow-up care is completed, and documentation is collected and filed.
  2. Assure residents receive their medications as prescribed and in a timely manner, including practice of assisting with self-administration of medications no more than one hour before or one hour after prescribed time.
  3. Monitor monthly Medication Administration Records to ensure proper recoding of administration of prescribed medications, as well as properly transcribing medical orders to the MAR sheet.  Track medication monitoring compliance and report medication error(s) to the resource coordinator using the appropriate reporting document
  4. Ensure proper security and safe use of medications, to include controlled substances; observations are documented
  5. Responsible for monitoring and evaluating urine drug screens, send to lab for confirmation
  6. Outreach:
    • Attend designated interagency meetings
    • Establish and maintain contact with treating physicians and psychiatrists
    • Act as liaison to treating physicians, psychiatrists, and pharmacy
    • Provide training presentations upon request
    • Accompany residents to appointments
  7. Coordinate with Program Manager to assure Medical Management of members for Continuous Quality Improvement.
  8. Partner with Director and Program Manager with on-site training and assistance to staff regarding the completion of the required medical documentation.

Mentoring and Training Responsibilities

  1. Work in collaboration with management to approach medical needs of members
  2. Participate in program plan reviews
  3. Participate in development of staff training
  4. Monitor the overall compliance of medical management requirements and accreditation standards of RHD and 5310 regulations and ensure RHD values and state regulations are followed.
Other Responsibilities:
  1. Need to be available to Unit Director, Program Manager during emergency
  2. Establish and maintain open communication with staff, as well as family of individuals receiving services.
  3. Promote a positive working relationship through interaction with community and agency organizations, as well as state and local entities
  4. Work with the Recovery Support Specialist to ensure that all appointments are adequately accounted for in the staff schedule
  5. Other duties may be assigned by your supervisor and/or Program Director to address the need of the program and residents.
Minimum Qualifications
  • Bachelor’s degree (degree in pharmacy studies, public health, nursing, psychology, sociology, counseling, social work, or education)
  • Current LPN license or CNA desired and accepted as applicants
  • At least 2 years’ experience working in a residential facility or mental health setting is preferred
  • Experience in/with mental health and substance abuse preferred.
  • Knowledge of psychopharmacology and medication administration records (MAR’s).
  • Good problem-solving skills, conflict resolution skills, knowledge of effective coping skills and the ability to work in flexible environments, including the community. 
  • Must possess a current valid State Driver’s License
  • Must pass criminal background check
