New Start II provides Substance Use, Mental Health and Withdrawal Management treatment to individuals suffering from chronic homelessness. This position provides overall therapeutic interventions, groups and individual sessions weekly with consumers. The Therapist will hold of caseload of 6-8 clients ongoing, while supporting person-centered, strength-based recovery options.
Complete necessary clinical documentation in accordance with all regulatory bodies
Complete state mandated trainings within 1 year of hire and maintaining necessary annualt raining hours per regulatory bodies
Hold one individual session per week for each assigned client, or more if needed foroversight, improvement, or treatment modification
Facilitate group therapy on scheduled days
Coordinate client discharge and aftercare planning
Evaluate client’s progress and complete clinical reviews with funding sources CBH & BHSI
Document in (EHR) Electronic Health Records according to regulatory bodies
Complete preliminary treatment plan within 24 hours, comprehensive treatment plan 30 days after and updated treatment plan every 30 days thereafter
Document groups, individual sessions, and all other significant contacts within 24 hoursof contact.
Complete discharge summary one week prior to discharge, or within 48 hours of client’s discharge.
Excellent communication and organization skills are required
Ability to be creative, work independently and contribute as a team member
Proficient in Electronic Health Record System (EHR)
Excellent time-management skills
Proficient at meeting deadlines
Must be able to work a rotating shift on weekends
New Start II is a short-term and long-term residential treatment facility that provides intensive drug & alcohol treatment, mental health, and withdrawal management support services to men. We service the chronically homeless population. The program also provides psychosocial support needed for the participant served to move towardsindependent living.
The program has a philosophy thatreflects the RHD Values of providing each person treatment in a safe,respectful and holistic environment. The goals andobjectives include a person-centered approach to treatment. The programsupports each individual to maintain sobriety, mental health stability, andengagement in a process to secure stable housing.
New Start II provides a variety of services,during the individuals 6-9 month treatment stay that includes individual and group counseling,psychoeducational groups, psychopharmacology treatment by a consulting psychiatrist, and case management services related the individualized needs ofthe participant i.e. housing, medical, and ongoing supports.
