Description/Job Summary

New Start II at Jackson Street is a long-term residential treatment facility that provides intensive drug and alcohol treatment and mental health support services to chronically homeless males. The program treatment design is an individualized level system, assisting residents in establishing psychosocial supports necessary for independent living. The Therapist will provide overall therapeutic interventions, groups, and individual session weekly with consumers. The Therapist will be responisble for a caseload of 2-4 clients  when necessary , while supporting person centered, strength-based recovery options.   On - Call  Therapist (20 hours)

Position Summary

The Therapist will provide overall therapeutic interventions, groups, and individual session weekly with consumers. The Therapist will be responsible for a  caseload of 2-4 clients  when necessary  while supporting person centered, strength-based recovery options.  

 
Essential Duties and Functions
Clinical Responsibilities:
  • Responsible for a case load of 2-4 clients ( when necessary )
  • Participate in and oversee all Clinical responsibilities as identified below; including but not limited to providing clinical coverage as scheduled and needed to ensure adequate coverage as per regulating bodies.
  • Complete necessary clinical documentation in accordance with all regulatory bodies
  • Complete State Mandates trainings within 1 year of hire and maintaining necessary annual training hours per regulatory bodies
Therapy and Case Management:
  • One individual session as per week per assigned clients, or more if needed for oversight, improvement, or treatment modification.
  • Facilitate group therapy on scheduled days
  • Facilitate educational seminars and or specialty groups as assigned
  • Attend clinical case consultation, staff meetings, trainings and supervision as scheduled
  • Provide crisis intervention
  • Assist in client aftercare planning and follow- up
  • Evaluate client’s progress and report funders/referral sources as requested
  • Communicate and document with outside agencies to ensure comprehensive services and schedule interagency meetings as deemed unnecessary
Documentation Responsibilities:
  • Complete client’s Bio psycho-social history within 14 days of client’s admission
  • Complete preliminary treatment plan within 24 hours, comprehensive treatment plan 30 days after and updated treatment plan every 30 days thereafter
  • Document Clinical Case Consultation every 60& 90 day of treatment
  • Document groups, individual sessions, and any other significant contact within 24 hours of contact.
  • Complete discharge summary within 72 hours of client’s discharge
  • Document aftercare follow –up as requested by Director
  • Review, approve and document pass requests, visitors, and phase checklists
  • Complete all necessary assessments, consents, PCPC/ASAM and other required documentation
  • Document and ensure continuity of care and foster a supportive nurturing environment for individualized growth
  • Document in DAP format
Additional Responsibilities:
  • On Call duties as assigned
  • Ability to be creative, work independently and contribute as a team member.

Requirements:
  • Excellent computer skills, fast learner
  • Excellent communication and organization skills required
  • Ability to be creative, work independently and contribute as a team member.
  • Proficient in Electronic Health Record System (EHR).
  • Ability to multi-task
 
Qualifications:
  • Master’s Degree in Social Services required.
  • 2 Years’ experience in D& A & Mental Health combined
  • Experience in D/A residential treatment preferred
  • Excellent writing, organization skills, running therapeutic groups
  • Document in DAP format
  • Computer skills, written and verbal communication must be strong
  • Ability to show compassion, and work as a team
  • Ability to apply learned skills (clinical applications)
  • Ability to utilize strength based approached language
  • Valid Driver's License
