A Team Leader is responsible for coordinating comprehensive services for individuals their program supports. The Team Leader works closely with program leadership, as well as clinical and direct support professionals to ensure that each individual supported is empowered to live the life that they want to live.
Essential Duties and Functions
Provides first-line supervision of Certified Peer Specialist within their program.
Actively participates in the person-centered planning process.
Completes Participant Intake Packets.
Assesses notes for compliance and accuracy.
Will be responsible for quality assurance of participant charts.
Bachelor’s Degree in a Human Services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology or
An associate degree with Three Year experience working to support individuals with intellectual disabilities/behavior health/dual diagnosis or
A High School Diploma with 5 years’ experience.
Current (CPRP) Certification.
