Description/Job Summary

The Program Coordinator performs all the case management services for individuals at HEART Services, a program servicing the needs of individuals struggling with chronic mental health and Chronic/Acute medical challenges.  The Program Coordinator will collaborate with the Program Director and the On-Site LPN in providing a seamless and efficient service delivery to the participants served.

Responsibilities/Duties

Responsibilities related to Clinical services to ensure the quality and timeliness of psychiatric intervention(s) as related to diagnoses, treatment plans, and progress of individuals

  • Complete and update all new intake documentation for residents.
  • Ensure that all annual and periodic assessments are completed and update.
  • Coordinate and participate in linkage meetings regarding resident treatment plans,
  • goals, quarterly reviews, and annual reviews.
  • Participate in concurrent reviews with Program Analyst from the Department of
  • Behavioral Health to review residents’ progress.
  • Complete all required documentation in compliance with agency regulations.
  • Responsible for entering all major incident reports into the HCIS system and reviewing
  • all incident reports with Program Director.
  • Review residents’ charts and update Recovery Plans and Functional Assessments every two months
  • Act as a liaison between HEART Services and outside agencies to ensure continuity of
  • care with residents.

Required Experience

A minimum of three (3) years’ experience in the field of Mental Health. Supervisory experience is a MUST.

Required Education

Minimum Bachelors in a Human Service Field.

Required Qualifications

  1. Experience developing therapeutic treatment plans.

  2. Proficient in Microsoft Office products with high aptitude for learning electronic programs

  3. Must possess a current valid State Driver’s License

  4. Must pass criminal background check
