Description/Job Summary

HEART Services is a licensed (Community Rehabilitation Residence) residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community.

HEART Services is seeking on-call, part-time and full-time Life Skills Professionals. The Life Skills Professional I (LSP I) is responsible for collaboratively and actively assisting and supporting the participants of HEART Services in the development of Life Skills, facilitating groups, and the establishment and attainment of goals identified in the individual Recovery Support Plan (RSP). The LSP I helps the participants of HEART Services attain overall wellness using strength based clinical model as tools for effective engagement.

Responsibilities/Duties

  • Serve as a member of HEART Service to provide support services to program participants living independently in the community.
  • Responsible for adhering to RHD and HEART Services values and beliefs and the principles of mental health recovery at all times
  • Responsible for working collaboratively with Clinical Manager and program participant to identify needs, services, and desirables.
  • Responsible for the development of Recovery Support Plans Goals with assigned participants at admission and at least 60 days thereafter, and supporting participants with RSP Goals daily
  • Become aware of the program participant’s strengths, and differing skills and abilities to determine the best approach and learning style that can help increase skill development.
  • Support/assist the program participant with the development and maintenance of necessary daily life skills to live independently within their community, i.e., cooking, shopping, household chores, seeking employment and education opportunities, medication management, interpersonal relations, utilizing community resources, vocational, budgeting, banking, accessing transportation, meal preparation, etc.
  • Responsible for providing crisis intervention when necessary.
  • Complete daily required documentation comprehensively and accurately, i.e., observation checks, MARs,  daily shift notes, mileage logs,  incident reports (if applicable), and vehicle incident reports (if applicable).
  • Training), and complete all corresponding documentation

Required Experience

  • 2 year of experience in mental health/social services field

Required Education

  • Must possess a high school diploma/GED

Required Qualifications

  • Excellent Communication skills (written and oral) including interpersonal skills
  • Ability to speak effectively before groups of individuals and communicate professionally with outside agencies.
  • Ability to manage multiple tasks or projects
  • Demonstrate good problem solving and conflict resolution skills
  • Ability to carry out duties without direct supervision
  • Demonstrate skills using Microsoft Office software and database management; ability to use the Internet as a resource and complete necessary documentation.
  • Ability to engage employees and participants in effective, therapeutic and recovery oriented interactions
  • Ability to support and implement RHD and Haldeman~Grant values and beliefs.
  • Possess a PA valid driver's license
  • Be 21 years of age or older

Program Summary

HEART Services is a licensed (Community Rehabilitation Residence) residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community.

HEART Services has the program capacity to serve 14 residents who present with a mental health diagnosis and an acute medical condition. Residents served demonstrate moderate to severe functional impairments that make it difficult to adjust socially and personally.

HEART Services partners with residents in their own apartments that are customized to their own liking, to learn the skills necessary to achieve success in an independent environment. HEART Services implements recovery oriented services that are focused on self-determination and incorporate empowerment, opportunity and choice.
