Description/Job Summary

Position Summary
 
The Therapist is responsible for delivering transgender affirming and trauma informed individual and group therapy to Morris Home members.  The Therapist will carry a caseload of 4 clients. 
 
Essential Duties and Functions
 
I.  Clinical Responsibilities:
         1.  Facilitate individual sessions with assigned members at least 1x weekly.
         2.   Facilitate group therapy at least 3x weekly.
         3.   Instruct educational seminars as assigned.
         4.   Attend Team Support Meetings, Recovery Plan Update meetings, and clinical supervision as scheduled.
         5.   Provide crisis intervention as needed.
         6.   Assist in member’s aftercare planning and follow-up.
         7.   Facilitate specialty groups as assigned.
         8.   Facilitate the therapeutic process.
        9.   Evaluate Members progress and report to funders/referral sources as requested through authorization re- authorization reviews.
         10. Prepare case consultation for assigned members at Clinical Team Meetings
         11. Determine resources needed to meet the individualized needs identified in the assessment process and coordinate community support activities.
         12. Facilitate necessary actions in assuring the implementation of comprehensive treatment plan
         13. Serve as an advocate for members surrounding needed services;
         14. Assist in identifying program needs and service gaps.
       
 
II. Documentation Responsibilities:
      1.  Complete psycho-social history within 14 days of Members admittance.
      2.  Complete initial recovery plan within 21 days of Members admittance; update recovery plan at least every  thirty days thereafter.
      3.  Document groups, education seminars, individual sessions, and any other significant contact with members daily.
      4.  Document Team Support Meetings (Case Consultations) at least every 90 days.
      5.  Complete discharge summary within 5 days of Members discharge.
      6.  Document aftercare follow-up within 7 days of discharge and again within 30 days of discharge.
      7.  Conduct screening/assessment interviews to determine Members appropriate level of care and treatment needs.       
      8.  Facilitate initial intake process as assigned.
      9.  Document all clinical contacts in electronic health records
 
 
III.  Other Responsibilities:
        1.  Participate in weekly team meeting, bi-weekly clinical team meetings, and monthly staff meetings
        2.  Attend weekly Community Meeting
        3.  Provide escort support for members  appointments as deemed therapeutically appropriate
        4.  Assist with developing and faciliating professional development trainings, both internally and externally.
 
 
Qualifications
 
 
Qualifications include:
  • Bachelors Degree and one year of experience or Masters in Human Services or related field preferred
  • Previous experience providing services to transgender and gender non-conforming clients
  • Knowledge of best practices in harm reduction and trauma-informed care
  • Signicant computer skills are required, including Microsoft Office and Electronic Health Records
 
Working conditions
 
This position is 40 hours a week, including at least one weekend day. This position may require occassional evening work. 
 
 
Reporting Relationships
 
Staff in this position will be responsible for supervising interns as required by director.
