Description/Job Summary

As a member of the management team of multiple programs, the Program Manager oversees and implements training needs, provide clinical coordination with internal and external stakeholders, manages outreach and communication with program partners, and analyzes practices and processes to offer and implement program improvements.  Reporting to the Unit Director, the Program Manager must be highly skilled in organization and nonprofit management, as well as have clinical insight and supervision skill sets.  The Program Manager must have a working knowledge of all required procedures of the FaSST/Connections unit and have a willingness to learn/implement new systems to improve the operations of this fast paced unit.



  1. Oversee training needs for FaSST/Connections.
  2. Manage and maintain outreach and communication to program partners.
  3. Provide clinical coordination to the FaSST/Connections unit and consumers
  4. Analyze, develop and implement best standards, Evidence Base Practices and opportunities to increase efficiency across the services in the program.
  5. Provide Supervision/managing/leadership to smaller units within a larger unit

Required Experience

  • Mastery of Targeted Case Management standards.
  • Proficiency in Microsoft Suites, email and other required databases
  • Proven supervisory experience and experience in a leadership role.
  • Ability to work in a fast paced environment.
  • The ability to handle a heavy workload and multitask effectively.
  • Familiarity with EHR system/ AVATAR (electronic health record)

Master degree or equivalent

Required Qualifications

Master degree or equivalent, TCM experience preferred, program management experience
