On Our Own II (a program of Resources for Human Development, Inc.) is a community-based program providing comprehensive residential and day services in five staffed homes to 9 adults with intellectual disabilities and significant mental health issues. Emphasis is placed on community and social integration, trauma based therapies, and mental health stabiliztion. On Our Own II is recruiting for a Program Specialist for our Adult Intellectual and Developmental Disabilities (IDD) Community Residential Homes in Lehigh, Northampton Counties.and Carbon Monroe Pike locations.
 
This is a full-time position (40 hrs/wk) with Monday to Friday hours from 8:30 am to 4:30 pm.  This position requires the ability to work flexible hours, which includes evenings and weekends, to meet individual and programmatic needs.  This position requires on-call responsibilities.
 
The Program Specialist position holds the responsibility of supporting health, safety, and growth of the individuals in the program; including medical requirements, programmatic concerns, individual finances, and incident reporting protocols.  The Program Specialist is responsible for the completion and coordination of the individual’s assessment, for the development, review, update and revision of the ISP, and for the implementation of goal plans.  This position requires well developed communication and writing skills.
 
 Essential Duties and Functions:

  • Create an environment that empowers individuals through the development of skills and coordination of resources and supportive services
  • Oversee the quality of programmatic, health and facilitative services by insuring that the needs of individuals served are met and necessary services and supports are provided
  • Schedule, chair and document ISP meetings and consumer reviews along with SC. Monitor annual ISP progress for all consumers, along with SC’s and other members of the consumer’s team.  Ensure all ISP’s are accurate and person-centered
  • Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually
  • Provide staff training as it relates to service delivery requirements and expectations as outlines in each consumers ISP.
  • Update staff in a timely manner every time service expectations change to ensure that all staff are properly trained and up to date on consumer plans.
  • Organize, maintain and update consumer files in accordance with the DPW 6400 regulations
  • Participate and coordinate licensing preparations and provider monitoring paperwork
  • Complete and review annual assessments and progress summaries for all consumers, all monthly and quarterly progress notes and lifetime medical histories
  • Ensure that all consumers’ medical, dental, psychiatric, and psychological services are scheduled in accordance with 6400 regulations and doctor recommendations
  • Maintain open communication with individuals, family members, day programs, Behavior Consultants, SC’s and all other outside entities
  • Coordinate all communication for the consumers with the Social Security Administration, Medical Insurance Companies, and other outside entities
  • Attend house meetings at the sites on a quarterly basis and conduct regular unannounced visits of all sites and share any necessary documentation
  • Serve as incident management point person, certified investigator, Medication Administration Trainer
  • Complete all documentation necessary as related to programmatic changes properly
  • Utilize Microsoft Word, Excel, and Power Point to complete reporting requirements and submit necessary data within a timely fashion

Qualifications

Education:
Master’s Degree in human services field plus 1 year experience working directly with individuals with Autism/MH/IDD,or
Bachelor’s Degree in human services field plus 2 years’ experience working directly with individuals with Autism/MH/IDD,or
Associates Degree, or a minimum of 60 credits in the human services field plus 4 years’ experience working directly with Autism/MH/IDD.

Skills/Knowledge:
  • Demonstrate a strong working knowledge of DPW 6400 regulations
  • Demonstrate good communication, organizational, leadership and time management skills
  • Demonstrate the ability to work independently with a minimum of supervision
  • Ability to effectively communicate both verbally and in written form
  • Ability to engage clients and staff in effective, therapeutic and healthy interactions
  • Ability to utilize and navigate Microsoft programs and applications
  • Possess a valid PA driver’s license
The Program Specialist will have direct contact with individuals that have co-occurring disorders including Intellectual and Developmental Disabilities (IDD), autism, mental health and/or behavioral disorders.  The Program Specialist will also be responsible to assist in the daily operations of each specific program as needed or necessary. The Program Specialist will be required to sit, stand and/or walk for extended periods of time and as necessary assist individuals in service with their daily needs.
