Job Description for Direct Support Professional (DRSPP-H)

Opportunity Place is an 18 bed Community Residential Rehabilitation Residence located in the

Powelton Village Section of Philadelphia.  The program serves 18 adult men who are recovering from Mental Illness and housing challenges.  The focus of the program is to provide a temporary residential setting where residents can establish both treatment and housing goals that suites their individual needs.   The Ideal candidate is forward thinking and plans on pursuing a career in the Mental Health/Human Services field.  This individual must have at least 1 years’ experience working with adults experiencing chronic persistent mental illness.  They must possess a High 

School diploma/GED (college experience welcomed), valid driver’s license, and pass a PA state 

Police background check.Computer knowledge (Microsoft Word) and the ability to learn and navigate an Electronic Health Record system is a must. This is a hands-on position that includes the following responsibilities:

Responsibilities/Duties

Job Duties/Responsibilities:

  • Responsible for assessment, planning, creating, implementing, monitoring and evaluation of participant’s daily living in accordance with the Recovery Model.  

  • Responsible for adhering to RHD values, policy and procedures, and Opportunity Place 

  • Responsible for following state and county CRR regulations 

  • Participates in evaluating needs of individuals served, and confers with Case Manager to plan for continuing additional services. 

  • Works in cooperation with other agencies in the service delivery system to ensure coordinated, continuous and effective services for the rehabilitation of the residents. 

  • Responsible for attending supervision, clinical meetings, monthly staff meetings,and mandatory training 

  • Responsible for the development of RSP Goals with residents 60 days and supporting participants with them daily 

  • Responsible for providing coaching to participants when necessary and appropriate 

  • Responsible for providing crisis intervention as defined in RHD/Opportunity Place Policy and Procedures Manual 

  • Responsible for monitoring participant medication according to Opportunity Place medication  policies 

  • Perform all cleaning duties as specified 

  • Operate agency vehicles in accordance with established policy and procedures 

  • Responsible for some meal preparation. Other duties assigned by management

  • If working on call you must be available to work ALL shifts.

Helping patients with daily activities, accompanying them to doctor appointments, cooking and serving meals, performing light housekeeping duties, running errands, and providing companionship
