Resources for Human Development, Inc. (RHD) is recruiting for a Program Specialist for our Adult Intellectual and Developmental Disabilities (IDD) Community Residential Homes in Lehigh and Northampton Counties.
 
This is a full-time position (40 hrs/wk) with Monday to Friday hours from 8:30 am to 4:30 pm.  This position requires the ability to work flexible hours, which includes evenings and weekends, to meet individual and programmatic needs.  This position requires on-call responsibilities.
 
The Program Specialist position holds the responsibility of supporting health, safety, and growth of the individuals in the program; including medical requirements, programmatic concerns, individual finances, and incident reporting protocols.  The Program Specialist is responsible for the completion and coordination of the individual’s assessment, for the development, review, update and revision of the ISP, and for the implementation of goal plans.  This position requires well developed communication and writing skills.

Responsibilities/Duties

  • Create an environment that empowers individuals through the development of skills and coordination of resources and supportive services
  • Oversee the quality of programmatic, health and facilitative services by ensuring that the needs of individuals served are met and necessary services and supports are provided
  • Advocate for consumer needs
  • Meet with consumer’s team to identify service needs and goals to ensure that such goals are accurately documented in the ISP
  • Schedule, chair and document ISP meetings and consumer reviews along with SC. Monitor annual ISP progress for all consumers, along with SC’s and other members of the consumer’s team.  Ensure all ISP’s are accurate and person-centered
  • Create, review and implement individual goals and data sheets based on assessment and ISP to monitor consumer progress
  • Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually
  • Provide staff training as it relates to service delivery requirements and expectations as outlines in each consumers ISP.
  • Update staff in a timely manner every time service expectations change to ensure that all staff are properly trained and up to date on consumer plans.
  • Attend and actively participate in all team meetings
  • Assist in the planning, writing and presenting of consumer case conferences and restrictive procedure plans

Required Experience

  • Demonstrate a strong working knowledge of DPW 6400 regulations
  • Demonstrate good communication, organizational, leadership and time management skills
  • Ability to engage clients and staff in effective, therapeutic and healthy interactions
  • Ability to utilize and navigate Microsoft programs and applications
  • Possess a valid PA driver’s license

Required Education

Master’s Degree in human services field plus on year experience working directly with individuals with Autism/IDD or Human Services, or
 Bachelor’s Degree in human services field plus 2 years’ experience working directly with individuals with AutismIDD or Human Services, or
 Associates Degree, or a minimum of 60 credits in the human services field plus 5 years’ experience working directly with Autism/IDD or Human Services.

  • Demonstrate a strong working knowledge of DPW 6400 regulations
  • Demonstrate good communication, organizational, leadership and time management skills
  • Demonstrate the ability to work independently with a minimum of supervision
  • Ability to effectively communicate both verbally and in written form
  • Ability to engage clients and staff in effective, therapeutic and healthy interactions
  • Ability to utilize and navigate Microsoft programs and applications
  • Possess a valid PA driver’s license
