Description/Job Summary

The CNA provides direct care services to group home residents.  The CNA performs tasks aimed at supporting the individuals’ needs and desires as part of a team

Responsibilities/Duties

  • Performs tasks in a manner that upholds the Values of RHD
  • Interacts with consumers in a manner that reflects compassion and understanding.
  • Interacts with peers, supervisors and the community in a manner that reflects a respectful, mature and professional  outlook
  • Performs tasks in support of the physical and mental well-being of the consumers
  • Provides consumers with supervision, support and assistance with all activities of daily living
  • Supports the ongoing health of consumers by assisting with self-administration of medication
  • Maintains patient stability by checking vital signs and weight; testing urine; recording intake and output information
  • Observes skin, behavior, general condition for changes; reports changes to RN immediately;
  • Completes all documentation as requested
  • Facilitates psychoeducational groups
  • Performs tasks to assure that program is in compliance with the policies and regulations of RHD and DBH
  • Other duties as assigned by management


Required Experience

3 yrs of Residential experience

1 year of Mental Health Experience

Required Education

HS Diploma

Required Qualifications

CPR and First Aid Training

Driver's License 

CNA Certification

Program Summary

NOVA II is a 22-bed residential program developed to address the needs of supporting people in recovery who are challenged with mental health issues and medically fragile complications. The program provides 24-hour nursing for optimum care and medical attention. The goal of the program is three-fold: 1) successful reintegration into a community-based living arrangement with a home-like environment; 2) development of each resident's maximum capacity for independent functioning; and 3) the development of a person-centered residential setting to enhance the quality of the residents' lives. NOVA II serves a five-county area and specializes in serving people with a history of long-term hospitalizations.
