Description/Job Summary

Position Summary

The Peer Support Specialist shall provide supportive services for justice involved individuals upon their re-entry into the community.   Individuals will typically have an existing behavioral health diagnosis or co-occurring mental health, substance abuse, and/or medical issues.  The Peer Specialist shall provide case management services to promote successful transition to the community and to reduce recidivism.
 
Applicants must be Certified Peer Support Specialists or willing to complete training.  Duties include assisting participants with their entry into supportive housing, completing benefit applications, linking to mental health/substance abuse treatment, wellness activities, job readiness, transportation assistance and supportive housing placement/counseling.  The Peer Specialist shall adhere to the principles of The Housing First Model.   A commitment to the RHD Values should be demonstrated as duties are performed.

Responsibilities/Duties

Essential Duties and Functions

  • Ability to build a rapport with individuals
  • Provide person centered services
  • Help individuals establish goals based on strengths, needs and preferences
  • Assist client in determining housing needs/plan, completing housing applications and applying for all available subsidies
  • Advocate with prospective landlords as needed
  • Coordinate with the Durham Housing Authority for resident placement
 Home Visits/Client Follow-up
  • Arrange weekly appointments with re-entry individuals
  • Encourage individuals to assume an active role in their care
  • Provide guidance with execution of personalized re-entry plan
  • Assist individuals with navigating community (riding the bus, accessing grocery store, health and safety)
  • Teach/model skills to maintain permanent housing
  • Practice active listening
 Maintain Client Records
  • Document case management activities daily
  • Maintain accurate documentation of services provided
  • Adhere to Federal, State and RHD policies and procedures
  • Report findings to State Director monthly

Required Experience

Must be able to demonstrate lived experience.

Experience working with homeless adults a plus.

Minimum 1 year of experience working in mental health, substance abuse field, working with homeless

Required Education

High School Diploma or GED

Required Qualifications

Must pass NC/Federal background check

Must have valid NC driver’s license, registration and insurance, and be willing to drive personal vehicles

Ability to demonstrate successful transition into the community after incarceration a plus

Program Summary

North Carolina RHD's Coming Home Program provides permanent supportive housing for justiceinvolved persons to address their unique service and support needs.  Thisprogram will target persons being released from prison or the local detentioncenter, as well as persons with frequent and current criminal justiceinvolvement who have a diagnosis of serious mental illness, substance use orco-occurring disorders and who are experiencing homelessness, have a history ofhomelessness or at risk of homelessness at the time of release.
