Description/Job Summary

Reporting to the Program Manager, the Direct Support Professional “DSP” is responsible for maintaining the safety and supervision of service recipients residing in community living homes operated by Resources for Human Development. The Direct Support Professional works as part of the Interdisciplinary Team to create and maintain a safe and healthy environment for the service recipients, while upholding RHD’s Values and DDDS Standards and DHSS Regulations.The Direct Support Professional fulfills the obligations outlined in the service recipient’s “Life Span Plan” (LSP) andPerson-Centered Plan (PCP), etc. The Direct Support Professional transports service recipients to medical appointments, recreation activities, day program services, etc. 

Responsibilities/Duties

The Direct Support Professional is expected to complete the following tasks, but  not necessarily limited to:

  • Participate in the development of the Life SpanPlan, Individual Support Plan, Person-Centered and Behavioral Support Plans for the service recipients and implement and document the plans as written based on the valued outcomes. Ensure that service recipients are treated with dignity and respect at all times.
  • Assist, plan, and coordinate community life activities and other leisurely activities based on the desires and needs of the service recipients.
  • Provide assistance to the service recipients incompleting their personal care needs, housekeeping, laundry, menus, food preparation, as well as, completing other activities of daily living.
  • Assist, plan and support the service recipients with scheduling and attending required medical, psychiatric and other appointments to maintain their good health. Ensure that all appointments are documented in the electronic record “THERAP” with any corresponding results from the appointments to include labs are uploaded as regulated. Ensures that medications ordered are must filled in a timely manner and transcribed on theMedication Administration Record and administered as regulated.
  • Responsible for providing care to the service recipients in the home due to day program closures and/or if service recipients are home due to medical or other reasons. The Program Manager will make every effort to ensure coverage but until coverage is provided, the Direct Support Professional is prohibited from leaving shift until a relief staff arrives and takes over. 
  • Meeting programmatic expectations by completing all required documents to include, but are not necessarily limited to,quarterly fire drill records, quarterly fire system checks, site environmental inspections, weight charts, monthly vitals, home and community notes on each shift, a billable note on each shift, etc.
  • Transport and assist the service recipients participate in community events of their choosing. These events can include,but are not necessarily limited to, social and/or spiritual activities, etc.
  • Attend and complete the on-site specific trainings and other related in-services as regulated by DDDS Standards, DHSSRegulations and RHD’s Policy to meet the needs of the service recipients.
  • Promote a positive self-image of all service recipients and advocate on behalf of and support the service recipients incommunicating their needs, wants, desires, dreams, etc.
  • Provide resources by ensuring that all service recipients have access to recreational and leisurely activities in the home and community to meet their needs.
  • Administer medications to the service recipients as prescribed after successful completion of the LLAM class and approval by the RHD Training Coordinator.
  • Secure emergency medical services for all service recipients when needed.

Required Qualifications

Ability to pass State Police, FBI checks, drug test

Adult and Child Abuse Registry clearances

Must be 18 years of age or older

PPD Clearance

Valid Delaware driver’s license and have an acceptable driving record
