Description/Job Summary

This is an exciting opportunity to join a community based case management/outreach team that was developed in compliance with the Governor’s Initiative for the Center of Excellence to address the opiate epidemic facing our communities.

Resources for Human Development (RHD) is seeking applicants for the Montgomery County Recovery Center (MCRC) for a community based Nurse Navigator position with a primary purpose to outreach to the community to help facilitate those in need into treatment.

The Nurse Navigator will be responsible for working with the local community to engage individuals with opiate use disorders to facilitate ongoing treatment and provision of other needed services to engage them and remain engaged. 


RHD is currently offering a $1000 sign-on bonus for this position; $500 after Orientation completion and $500 after 90 days of employment!


Part-TIme (approx. 20 hours/week)

Schedule: Flexibility to work on weekdays between the hours of 6:00am - 2:00pm.


Responsibilities/Duties

  1. Assist clients in scheduling and coordinating their medical appts
  2. Schedule and participate in outreaches with medical providers
  3. Maintain open communication with all staff
  4. Run each client’s PDMP as part of the DHS spreadsheet when clinically appropriate.

Required Experience

Knowledgeable about Substance Use Disorders and MAT (Medication Assisted Treatment)

Required Education

Registered Nurse or Licensed Practical Nurse/ Maintain Current License

Required Qualifications

  • Must have knowledge of Addiction and MAT (Medication Assisted Treatment)
  • Case Management experience a plus
  • Ability to navigate multiple systems and be able to multitask
  • Experience with clinical assessments and evaluations
  • Must have reliable transportation
