The Program Specialist functions as a member of the administrative team for RHD Boston working to ensure the agency meets high quality standards in the development and implementation of Individual Service Plans (ISP's). The Program Coordinator works as a resource to managers and staff in the development of meaningful goals and activities aimed at enriching the lives of the individuals served by the agency.
ISP Planning Meeting
Conducts Satisfaction Surveys with individuals and their significant family members and or guardians
Schedules and facilitates Pre-ISP meetings prior to the formal Annual Review Meeting
Reviews paperwork related to ISP goals prior to the Pre-ISP meeting
Assists with the development of ISP goals and associated strategies and data collection methods
Monitors ISP Submissions
Reviews documents entered in to HCSIS (State electronic healthcare record)
Monitors deadlines and requests for documents from Service Coordinators
Tracks all the ISP paperwork process
Provides technical assistance where needed
Periodic Review of Individual Records
Reviews records every six months for information related to goals, strategies and progress
Checks records for necessary documentation, provides feedback to managers
Prepares reports on the status of ISP documentation
Participates in the training of managers in the expectations of the ISP process and documentation
Assists with the identification of training needs for staff and managers related to the ISP
Minimum of two years experience working with individuals with ID/DD/Autism
Experience with teaching skills to individual
Familiar with HICSIS and ISP expectations
Bachelor's degree in related field preferred, Associates degree in related field and years of experience may be substituted
Training in person centered planing and trauma informed care
Applicant must have a driver's license and be able to travel to programs throughout the Boston area

Salary Range: $16 - $23 per hour
