The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD’s multi-faceted
programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness.
The Mission of Outside the Lines Studio (OTL) is to provide individuals with developmental disabilities/intellectual disabilities/ASD with the tools and materials to create art and the supports to define themselves as artists. OTL works with artists with
a range of disabilities, in particular people whose challenging behaviors or health issues make them poor fits with more traditional day services. Many of the artists' works are included in private collections throughout New England. Artists have been successful
in local and national juried exhibitions. RHD is hiring a full-time Case Manager for the OTL Program.
The program has a strong commitment to helping individuals explore and become active members of the local and surrounding communities.
Individuals who express and interest in exploring paid work opportunities as a future goal are provided with opportunities to work on prevocational skills in the center and the community
Essential Duties and Functions
Oversees a case load of 6-8 individuals (this may change based on day program census)
Participates in meetings with the individuals and the team to identify goals and strategies for reaching those goals identified in the Individual Support Plan (ISP)
Reports on ISP goal progress through progress notes and synthesizes data for yearly ISP meetings.
Attends ISP meetings as a representative of OTL
Works indirectly 1 day per week in the RHD office to prepare ISP materials, write progress notes, work on group protocols, and/or assist with recording and filing data
Writes progress notes as defined in the ISP for individuals on case load and submits to Clinical Coordinator for review
Prepares ISP documents for individuals on case load
Records data on appropriate forms to track individuals progress on ISP goals
Attends monthly Case Manager meetings to discuss case load and emerging issues
Attends regular staff meeting
Provides direct support to individuals in the program 4 days per week (including personal care and behavioral support)
Provides transportation to individuals to and from the program (program vehicle provided)
Teaches and assists in classes and activities designed to address the needs and objectives of participants
High School Diploma/GED required. Associates or Bachelor's preferred
Minimum of one year working with individuals with IDD/DD/ASD preferred
Valid Driver's License
Preferred Specialized Knowledge
Familiar with DDS reglations and support systems
Understanding of Human Rights
Familiar with Positive Behavior Supports (PBS)
