Description/Job Summary






INTAKE COORDINATOR:


Assisting in the oversight of the admissions process.

Work with individuals seeking services one-on-one tocomplete documentation and intake requirements.

Take Intake calls over the phone. Follow up withhospital discharge.

Enter demographic information in Avatar. Open chartswith proper episodes.

Verify insurances. Save EVSs in proper shared drive.

Schedule Assessments.

As Intake Coordinator work in conjunction with theClinical team and falls under the umbrella of Care Coordination.

Communicates with referralsources and families, in a timely manner, to obtain necessary information anddocuments.

Strong verbal and written communication.

Detail-oriented and interpersonal and organizational skills.

Able to Multi task.


Additional:

Using Avatar to input client date for FinancialEligibility. (IE. Social security numbers)

Assisting in scheduling of new Intakes.

Working one-on-one with clients as needed to completenecessary admissions paperwork.(IE Insurance)

Working collaboratively with other members of theclinical team to disseminate client information where applicable.

Update tracker reportdaily.

Provide weekly summary onall calls.

Scanning Intake packets by next day after signature.

Billing errors- complete FE date(IE missing Socialsecurity, DOB, and policy numbers)

Attend meetings.


Qualifications:

  • Preferred Bachelor’s Degree insocial service, medical, or professional field preferred

  • Proficient knowledge of MicrosoftOffice (Excel, Word, PowerPoint) required.

  • Ability to express complexinformation in a clear and concise format.

  • Ability to present information tovarious levels of understanding.

  • Intake/Admissions experiencepreferred.

  • Strong critical thinking,analytic and problem-solving skills.

  • Must be very organized.

    Must have the ability to work independently as well as work effectivelywith affiliated teammates.

Lower Merion Counseling Services has been providing a broad range of mental health and substance abuse services for over 50 years. Lower Merion Counseling & Mobil eServices offers help to people whose lives have been affected by mental illness and/or substance abuse.
