Assess needs and service eligibility of consumers/potential consumers.
Provide promt screening and referral to appropriate treatment and supports, and associated record and data management.
Within 2 business days of first face-to-face contact, assist with benefit applications and complete registration paperwork. Assist with othe resource/benefit acquisition and coordination as appropriate (Medicaid, housing, social services, transportation, ect).
Maintain knowlege of County services, community resources and supports.
Provide information and referral for behavioral health and other human services for other RHD agencies.
Track and coordinate the behavioral health commitment process.
For individuals needing ongoing minimal support, ACM will:
Regularly monitor and support the participant's needs.
Provide ongoing review of eligibility and need for other services and maintaing monthly/weekly EVS file.
ACM will maintain monthly tracker report for all MHX due on the 5th of every month.
If a problem arises with the participant's benefits, the ACM will assist in addressing these issue with the person.
The ACM will receive copies of mail sent to the participant regarding benefits and help the individual to respond accordingly.
If the individual is presenting with an increase continual need, the ACM will make referrals to higher level of service as appropriate.
