The function of the Team Leader is to oversee the daily clinical operations and service delivery of the program. The Team Leader provides supervision to direct care staff and consults with other supervisors as needed. The Team Leader ensures the implementation and fidelity to the model by assuring that the team develops and implements best practice treatment modalities while assumes a rehabilitative approach to recovery. The Leader is supervised by the ACT Clinical Manager.
Provide on-call supervisor coverage as scheduled.
Lead daily meetings to review all consumers.
Supervise the development of individual recovery plans
Supervise the provision of recovery counseling and individual treatment services to program consumers.
Provide individual and/or group staff supervision.
Assist with the completion of probation and annual staff evaluations
Participates in supervisor on-call rotation for ACT program
5 years of related experience
A Master’s degree in Social Work or Counseling
License required (LCSW, LPC)
Organizational skills; ability to complete documentation timely and accurately
The ACT Program provides community based services to individuals with severe and persistent mental illness. Their mental illness may also be accompanied by a substance abuse disorder and/or a developmental disability. ACT is an evidenced based, recovery oriented service delivery model that provides a holistic, multidisciplinary approach of consumer care. ACT support services are not time bound. The primary goals of the program are to lessen or eliminate the debilitating symptoms of mental illness each consumer experiences, minimize or prevent recurrent acute episodes of the illness and to enhance quality of life and functioning.
