The function of the Registered Nurse (RN) is to participate as a full team member and ensure medical needs, physical health issues, and medication issues are integrated into treatment. The RN attends psychiatry appointments and ensures all needed follow-up is completed.
Implement, maintain, and coordinate the medical treatment for ACT consumers.
Perform medical functions regarding basic health, medical assessment and education.
Complete initial and annual Health Assessments.
Obtain physician's orders for prescription(s) for consumers and get them filled:
Insured consumers: through a pharmacy of consumer’s choice
Uninsured consumers: through JPHSA, patient assistance programs, and/or sample medication
Ensure medication order sheets are signed by the staff MD.
Record injection medication on the mediation administration record.
Inform team members of any needs consumers have for medication or laboratory work.
Provide locked security for medications and set aside a private designated area for set up of medications.
Administer medications and provide injections to consumers as directed by doctor
Check consumer medication trays and fill them as needed
Provide services at times and locations convenient to consumers.
Provide on-call, after-hour support to consumers as scheduled.
Provide face-to-face services in the consumer's home, in public places, wherever necessary.
Make contact with consumer within 5 business days of receiving a referral.
Ensure consumers have access to and utilize the ACT after-hours on-call staff for emergencies or after-hour support.
Provide on-call coverage as part of staff rotation.
Provide medical education to consumers.
Maintain confidentiality for consumers.
Provide transportation to medical appointments or other agency events.
Provide assistance with family (in home family education and relationship building)
Utilize Motivational Enhancement Therapy (MET) and Stages of Change techniques to engage and provide services to consumers.
Utilize recovery-based, person-centered treatment approaches and harm reduction strategies with consumers.
Assume lead roles with designated consumers. Develop, coordinate, and implement treatment, rehabilitation, and support services for these cases.
Provide relapse prevention training and on-going education as outlined in the treatment plan of consumers.
Provide assertive outreach when necessary to engage consumers.
Assure continuity of care when consumer is in the hospital, jail, respite, etc.
Participate in on-site discharge planning for new ACT referrals preparing to leave an inpatient unit to become a consumer of ACT.
Engage in collateral consultation and service coordination planning meetings.
Submit accurate and on-time chart documentation, reports, and mileage reimbursement documentation.
Document telephone and face-to-face contacts with consumers.
Document all collateral contacts and community-based activities.
Perform quarterly self-audits of all nurse and psychiatrist documentation to ensure accuracy.
Maintain up-to-date consumer charts, statistical forms; and other information and documentation as required.
3-5 years working as a RN with persons with severe mental illness and substance use/abuse issues
ASN required, BSN preferred
Must be a Licensed Registered Nurse
Must have a valid drivers license and car insurance
The ability to make ethical clinical decisions in demanding situations
The ability to interact professionally with clients, their families and other professionals
Organizational skills; ability to complete documentation timely and accurately
RHD's Louisianna ACT Program provides community based services to individuals with severe and persistent mental illness. Their mental illness may also be accompanied by a substance abuse disorder and/or a developmental disability. ACT is an evidenced based, recovery oriented service delivery model that provides a holistic, multidisciplinary approach of consumer care. ACT support services are not time bound. The primary goals of the program are to lessen or eliminate the debilitating symptoms of mental illness each consumer experiences, minimize or prevent recurrent acute episodes of the illness and to enhance quality of life and functioning.
