Description/Job Summary

The Assertive Community Treatment (ACT) team Mental Health Counselor is a licensed mental health clinician delivering community-based therapy services to participants of the ACT program.  The interventions of the community team are designed to respond to the strengths and needs of individuals participating in the ACT program diagnosed with a serious and persistent mental illness.  

The responsibilities of the team clinician include; assessment of individuals referred for therapy and the provision of clinical direction for services planning and treatment goals, conducting individual therapy, utilizing member in vivo experiences to maximize learning and use of skills and developed insights, participation in team and individual case management activities, facilitation of treatment team meetings, discharge planning.  

The Waterloo ACT program provides services in a 30 mile radius of Waterloo. The ACT Mental Health Clinician is a mobile staff position dedicated to at least 85% of the work time to direct community-based work with clients in their home or community-based settings. This position will also support the operation of the program on a rotating on-call, evening, and weekend schedule


  • Provide services at times and locations convenient to clients. 
  • Provide on-call, after-hour support to clients as scheduled.
  • Provide face-to-face services in the client’s home, in public places, wherever necessary. 
  • Ensure clients have access to and utilize the ACT after-hours on-call staff for emergencies or after-hour support.
  • Utilize evidence-based interventions and therapeutic techniques to address consumer’s mental health symptoms 
  • Provide relapse prevention training and on-going education as outlined in the treatment plan of clients.
  • Develop treatment plans, early intervention plans, and recovery action plans with clients.
  • Perform crisis intervention.
  • Provide assertive outreach when necessary to engage clients.
  • Assure continuity of care when client is in the hospital, jail, respite, etc.
  • Actively participate in the engagement of new clients into the ACT program.
  • Maintain up-to-date consumer charts, statistical forms; and other information and documentation as required.

Required Experience

  • Minimum of 3 years of prior experience

Required Education

  • A Master's degree in Social Work, Counseling, or Occuplational Therapy

Required Qualifications

  • Iowa licensure in the field of LISW, LMSW, OT, LMHC, LMFT required, or the ability to become licensed in the first 6 months of employment. 
  • Valid Iowa Driver's license
  • The ability to make ethical clinical decisions in demanding situations
  • The ability to interact professionally with clients, their families and other professionals
  • Organizational skills; ability to complete documentation timely and accurately

Assertive Community Treatment (ACT) is an evidence-based, best-practice model of service provided through an interdisciplinary team to ensure a recovery-oriented system of care. ACT is available 24 hours a day, seven days per week, and is prepared to carry out a full range of treatment functions in the participants chosen community.  RHD Iowa ACT in Waterloo serves program participants in a 30-mile service territory.
