Description/Job Summary

RHD is seeking a temporary Case Manager to assist our program for a 60 day assignment.

RHD is hiring a temporary Case Manager to assist with our caseload!  This is an opportunity to build a career serving those with mental health/substance abuse issues and potentialy become a permanent position. The Case Manager will work to assure that direct services are provided in collaboration with clients, significant others, and collateral resources. This team member will assist adults with a diagnosis of a serious and persistent mental illness to gain access to needed resources such as financial, housing, medical, social, vocational, educational, leisure and mental health services.

Responsibilities/Duties

Identify and respond to the needs of the consumers through weekly face to face appointments

Engage individuals in a trusting and supportive relationship, providing emotional support within professional boundaries.

Assist the individual to reach the goals identified in the Treatment Plan

Maintains required records accurately, comprehensively and in a timely manner. Keeps required consumer data up to date and submits paperwork as per program policy.

Participate fully as a member of the team in staffing and other meetings, offering relevant input and feedback and accepting such from other team members.

Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, and compliance with all regulatory requirements.

Ensures the confidentiality of all client and/or employee-related information.

Required Experience

3 years Preferred

Required Education

BA/BS in Human Service Field

Required Qualifications

Valid Iowa Drivers License
