The Primary Care Lead NP is a leadership position that fulfills the role of clinical team leader, mentor, supporter, and clinician resource. In addition, the Lead NP will be responsible for continually assessing and retooling clinical workflows in conjunction with Center Directors and other site leadership in order to maintain effective, efficient practice. The Lead NP position also serves on site and network level committees including the joint primary care/ operations committee and the primary care team leaders committee in order to represent the concerns of their fellow NPs at the site level, and bring input into decision-making regarding policy development, standardization of network protocols, CQI priorities, and other issues influencing the experience of clinicians at FPCN. This position is expected to add an additional administrative burden to the clinician’s schedule of 3 sessions per week (approx. 12 hours) that will be dedicated to the above-named activities. This position will be re-evaluated at 1 year to assess fit and role continuity.
Assurance that clinical setting is functional and equipped for daily patient care.
Work with clinical staff and Center Director (including Front Desk, Medical Assistants, and Nursing, Nurse Practitioners) to maintain or exceed expected weekly / monthly productivity.
Ongoing monitoring and adjustment of processes responsible for smooth and effective patient flow.
Provide clinical leadership and mentorship for Nursing and Nurse Practitioner practice
Share new clinical policies / procedures
Chart reviews as needed for audits and QI
Patient care issues
Promote Quality Improvement processes, power cycles, and support network improvement initiatives and quality measure goals.
Facilitate morning huddles and assist in running clinical staff meetings with focus on primary care team issues and functioning.
Encourage and promote teamwork. Nurture individual team member’s strengths and abilities.
In collaboration with the RN, oversee Medical Assistant functions including: ordering medical supplies, vaccines, inventory, clinical office needs, and daily patient flow.
Work with Center Director (Operations) and Director of Primary Care to cover unexpected clinical absences. Assist with coverage issues as needed.
Work with Medical Assistants and nursing staff daily to problem solve clinical workflow issues.
Report staff concerns to Director of Primary Care. Routine supervision meetings, at least twice a month, with Director of Primary Care and Operations Site Director to facilitate smooth communication and promote excellence in patient care services offered on-site.
Resource and liaison for other members of healthcare team (Behavioral Health, Outreach, Social Work etc.) Work in conjunction with Center Director to promote positive team dynamics through regular meetings and staff supervision.
Hold regular supervision meetings with nurse practitioners and RNs, promote productivity, identify strengths and develop individualized plans to address areas of improvement or development, or desired acquisition of skills. Supported by Primary Care Director.
Collaborate with Director of Primary Care and Center Director regarding clinical supervision of clinical staff. Ability to request meetings with both Directors and clinical staff as needed to address deficiencies/complaints or clinical concerns.
Report clinical staff concerns to Director of Primary Care.
Input into disciplinary action regarding clinical staff. Initiate process when indicated. Consistent and timely documentation and communication of problems regarding primary care staff.
Provide input for MA, front desk performance reviews in collaboration with Center Director.
Perform annual performance review of RN in collaboration with Director of Primary Care.
Assist in interviewing and hiring of primary care clinical staff in collaboration with Director of Primary Care and Center Director.
Network with other Primary Care Site Directors; meet as needed to ensure standardization of clinical policies and procedures between sites
Contribute to the revision and approval of policies, protocols, and network clinical guidelines.
Attend meetings / conferences regarding clinical management or other topics that help promote clinical practice.
Participate in continuing education activities that will enhance management and administrative knowledge and skills and contribute to professional growth and leadership abilities.
Perform other duties as assigned.
Minimum 2 years as a Nurse Practitioner / Demonstration of clinical competence
Salary commensurable with experience
Physical and emotional ability to perform the duties as described above.
Proficiency in computer skills and electronic health record.
Leadership and Management Skills or willingness to train.
