Family Practice & Counseling Network (FPCN), a network of federally qualified community health centers, is seeking a full time
Psychiatric Mental Health Nurse Practitioner (PMHNP) for our Abbottsford Falls location, but we would consider a Part Time candidate who has a passion for community based health center population. The position is based within primary care
working with a multidisciplinary team.
FPCN practices a nurse-managed, integrated model to deliver care. The Network promotes resiliency and well-being among patients, staff and surrounding communities. Utilizing a multidisciplinary approach, FPCN provides a variety of services under one roof, including primary care, behavioral health, dental, cardiology, podiatry, diabetic education, prenatal care, physical therapy, family planning, a pharmacy, and more.
The PMHNP works within Primary Care under the Integrated Behavioral Health (IBH) Department. At all of FPCN’s centers, IBH has Behavioral Health Consultants (BHCs) and Psychiatric NPs to support the care provided to our primary care patients. The goal of the PMHNP is to increase support of the medical and behavioral interventions by assisting with medication management. Patients are taught behavioral strategies to help them adhere to medical regimens, new coping mechanisms, stress reduction strategies, and pain and/or disease management techniques.
The ideal candidate should be interested in working with a multidisciplinary team which consists of the PCP (Nurse Practitioners), BHCs (LCSW or Psychologists), RNs, MAs, Social Work, etc. Because this is a medical setting, flexibility is a must and having some knowledge of medical conditions is a plus.
Team members are expected uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.

Family Practice and Counseling Network, a program of Resources for Human Development, is a network of community health centers providing comprehensive primary care, dental and behavioral health services. The Network consists of four health centers in Philadelphia – Abbottsford-Falls Family Practice & Counseling in the Northwest, the Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University in the North, the Health Annex in the Southwest, and Annex West Health Center in West Philly housed in Action Wellness building (formerly Action AIDS); a convenient care clinic in Philadelphia – Fast Family Care. Today the Network serves more than 20,000 patients annually and is an NCQA certified Patient-Centered Medical Home.
