On- Call ADL Counselor
 Job Description

Job Duties/Responsibilities:

  1. Resident Chores/House Maintenance
    • Support residents in maintaining the physical plant
    • Support residents in completing their household tasks
    • Maintain the building repair log
    • Support residents in maintaining daily schedule.
  2. Food and Household Goods
    • Assist, monitor and support the cook in meal planning, food inventory, the creation of a grocery shopping list, and grocery shopping as assigned by the Director.
    • Support, assist and monitor meal preparation
    • Support implementation of Nutrition Program, if applicable
    • Monitor and assist at meal time
    • Monitor the food and household budget
  3. Medication and Health Management
    • Assist residents in securing prescriptions
    • Log new medication in medication log book
    • Monitor the resident’s self-administration of all medication and log in medication log as assigned by Director
    • Assist residents with scheduling medical appointments and follow up on medical recommendations when necessary
    • Collect and document collection of residents’ urine samples as assigned
  4. Educational:
    • Distribute and collect initial and discharge Consumer Satisfaction Survey
    • Assist residents in conflict resolution
    • Provide crisis intervention
    • Participate in program milieu
    • Attend and/or facilitate educational groups and meetings ( e.g., meal planning, housing, budgeting and Community Meeting)
    • Actively support residents in facilitating socialization and recreation activities
  5. Program
    • Document educational groups and any other significant resident contact
    • Conduct, facilitate and document fire drills as assigned by the Director
    • Participant in Case Consultation Meetings, Staff Meetings and supervision as assigned by the Director
    • Inventory each resident’s possessions upon admission and discharge
    • Assist residents in securing all benefits for which they are eligible (e.g. Welfare)
    • Read log daily an document in log at the end of each shift
    • Escort residents to meetings, etc. as assigned by Director
    • Provide emergency coverage for next shift
    • Coordinate referrals to housing resources as needed
    • As assigned by Director to ensure continuity of care
Job Requirements/Minimum Qualifications:
  • High School Diploma or G.E.D, Associates Degree or Bachelor’s Degree
  • Minimum 1 year experience working in Drug and Alcohol and/Mental Health setting
  • Valid Driver's License
