How Will You Make An Impact

RHD's Jefferson Street Group Home is is looking for full-time and part-time Registered Nurses to collaborate with the Service Line Director and Unit Director, in assuring that program is compliant with all LLAM regulations, DSAMH regulations, CARF standards, and Long Term Care regulations. This is a great opportunity to work in Behavior/Mental Health with focus on providing centered/individualized medical and mental healthcare to our eight residents and help the residents thrive and become more independent.

Full-time (40 hours per week) opportunities are available Monday-Friday (schedule is flexible) with an annual salary of $60,320.

Part-time opportunities are available Monday-Friday during evening shifts, and/or weekend day and evening shifts (schedule is flexible) with a pay rate of $29.00 per hour.

  • Maintain appropriate standards and documentation regarding medication storage, controlled substances and medication errors.
  • Participate/collaborate with psychiatric physician, resident and Unit Direct, and clinician in weekly treatment team meetings
  • Review MAR's weekly for completeness, compliance and possible errors; record changes in MARs and review cycle medications.
  • Communicate any and all problems regarding medication (including documentation) to the supervising nurse immediately.
  • Collaborate with treatment team in supporting physical and mental health of Residents.
    • Attend weekly treatment team meetings with resident and team.
    • Review labs/notes as need to assure appropriate follow up on health issues.
    • Work individually with residents as needed to address particular health needs (i.e. diabetes, dietary challenges).
    • Attend medical appointments with clients as needed or as requested to provide assistance and support.
  • Provide staff and residents with instruction on specific health topics on an as needed or as requested basis.
  • Communicate with pharmacy on an ongoing basis to assure residents have appropriate medications throughout the month.
  • Communicate with medical personnel in the community to advocate for consumers' health care needs and to provide follow up as needed
  • Provide assistance to staff and residents in emergency situations.
  • Will provide care to consumers in a manner that reflects RHD Values.
  • Additional Responsibilities as assigned.
  • Provide LLAM training (LLAM registered nurse trainer, if you don’t have will help get registered) to all current and new staff.

Required Experience

  • Must be a Licensed Registered Nurse with at least 2 years of experience

Required Education

  • ASN required, BSN preferred

Required Qualifications

  • Must be a Licensed Registered Nurse
  • Valid Driver's License required
