Jefferson Street Group Home is part of RHD Delaware Group Homes that provides 24/7 assistance for adults with chronic mental health diagnoses. Compassionate care and outstanding services to our eight residents are core values of this program.

The program is looking for full-time/part-time Registered Nurses. Day shifts, evening shifts, and weekend shifts are available. Compensation 29.00

The full-time position offers a great opportunity to work with Behavioral and Mental Health in a collaborative role with the psychiatric doctor and the Unit Director and residents, especially if interested in being a psychiatric nurse practitioner to gain invaluable work experience.

Responsibilities/Duties

  • Collaborate with Service Line Director and Unit Director, in assuring that program is compliant with all LLAM regulations, DSAMH regulations, CARF standards, and Division of Long Term Care regulations.
  • Provide LLAM training (one LLAM registered as a trainer) to all current and new staff.
  • Maintain appropriate standards and documentation regarding medication storage, controlled substances and medication errors.
  • Review MAR's weekly for completeness, compliance and possible errors; record changes in MARs.
  • Communicate any and all problems regarding medication (including documentation) to the supervising nurse immediately.
  • Collaborate with treatment team in supporting physical and mental health of Residents.
    • Attend weekly treatment team meetings with resident and team.
    • Review labs/notes as need to assure appropriate follow up on health issues.
    • Work individually with residents as needed to address particular health needs (i.e. diabetes, dietary challenges).
    • Attend medical appointments with clients as needed or as requested to provide assistance and support.
  • Ongoing Education
    • Provide staff and residents with instruction on specific health topics on an as needed or as requested basis.
  • Community Liaison
    • Communicate with pharmacy on an ongoing basis to assure residents have appropriate medications throughout the month.
    • Communicate with medical personnel in the community to advocate for consumers' health care needs and to provide follow up as needed.
  • Emergency Assistance
    • Provide assistance to staff and residents in emergency situations.
  • Will provide care to consumers in a manner that reflects RHD Values.
  • Additional Responsibilities as assigned.


Required Experience

  • Must have at least 1 year of experience

Required Education

  • Minimum education: Associates Degree

Required Qualifications

  • Must be a Licensed Registered Nurse
  • Valid Driver's License required
