Description/Job Summary

The DE ICM team provides non-traditional community based mental health treatment to promote the recovery and community integration of 100 adults with serious mental illnesses and co-occurring substance use disorders living in the community (Sussex and Lower Kent County) to reduce use of ERs and inpatient hospital stays. The case manager is an integral part of a multi-disciplinary team of mental health clinicians. The case manager provides ICM treatment services such as wellness self-management counseling, integrated dual disorder treatment, daily living skills, and general case management.  This is a field-based position and requires at least 75% of services to be provided to participants in their homes, shelters, places of employment, vocational and education settings and other community settings.  CADC required.


Working Conditions:

  • Necessary Travel-must have a current driver’s license and vehicle insurance in good standing.Use of personal vehicle is required.May have to drive in inclement weather.
  • Unusual Work Hours-hours as assigned by Program.Scheduled holiday.
  • Unusual Environmental Conditions-Inside and outside work area.Environment may be noisy at times.Necessity to practice universal precautions at all times due to possible exposure to infectious disease.
  • Physical Requirements-
    • Ability to sit approximately 50-75% of the time
    • Ability to stand approximately 25-50% of the time
    • Ability to climb stairs approximately 10-30% of the time
    • Ability to lift up to approximately 20-25 lbs
    • Ability to bend approximately 10-30% of the time
    • Ability to stoop approximately 10-15% of the time


Responsibilities/Duties

  • Develop recovery plans in collaboration with the participant and participant’s support system
  • Involve family and other natural supports in the participant’s treatment with the ICM team and provide psycho-education to supports as needed
  • Complete comprehensive assessment, crisis plans progress notes, and other required paperwork as required
  • Provide individual employment and vocational counseling
  • Assess and provide supports to consumers to address needs for problem-solving, wellness self-management, housing, education and vocational training, social supports, and employment.
  • Provide 24-hour on-call services and crisis intervention, on rotating basis
  • Provide 75% percent of treatment contacts in the community
  • Provide culturally competent services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability,  handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex
  • Maintain confidentiality and adhere to HIPAA
  • Perform other duties as required by Team Leader/Unit Director

Required Experience

2 years experience

CADC certified

Required Education

4 year Bachelors degree

Required Qualifications

  • Fluency in Spanish preferred
  • At least Bachelor’s Degree in a Human Service Area
  • At least one year of relevant experience
  • Ability to pass a drug screen and background check
  • Valid driver’s license and car insurance
  • Highly organized andself-motivated
  • Proficient in computers
  • CADC required
