Description/Job Summary

This position will support individuals who receive services on an Assertive Community Treatment (ACT) Team and live in Supported Living. 

Responsibilities/Duties

  • Work with consumers to assist them with building skills to promote recovery and self-sufficiency
  • Involve family and other natural supports in the consumer’s treatment; provide psycho-education as needed
  • Provide formal individual and group peer support utilizing Illness Management and Recovery (IMR) and Wellness Recovery Action Planning (WRAP)
  • Provide informal wellness management and recovery strategies, including, but not limited to recovery strategies, stress management, psycho-education, symptom management, and social skills
  • As needed assist with consumer income support by escorting consumers to appointments to apply and recertify health insurance (Medicaid, Medicare, private insurance) and income (food stamps, SSI/D, etc.)
  • Link consumers to peer support and  self-help recovery resources
  • Coordinate with ACT Team
  • Complete assessments, progress notes, and significant data in required time periods
  • Provide culturally competent services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability,  handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex
  • Maintain confidentiality and adhere to HIPAA
  • Any other duties as required by Team Leader or Unit Director

Required Experience

  • Must self-identify as an individual with a serious mental illness who is currently or formerly a recipient of mental health services. As a peer in recovery from mental illness and/or co-occurring serious mental illness and substance abuse, the peer must be willing to utilize recovery experience to engage and support consumers.

  • Strong knowledge of wellness, recovery and self-help
  • Ability to work effectively with diverse team and remain highly organized and self-motivated
  • Proficient in computers  
  • Ability to pass a drug screen and background check
  • Valid driver’s license and car insurance

Program Summary

The Assertive Community Treatment (ACT) Team is a service provided by an interdisciplinary team that ensures a recovery oriented system of care is available 24 hours a day, 7 days per week and is prepared to carry out a full range of treatment functions wherever and whenever needed. The program employs master’s and bachelor’s level counselors, registered nurse, substance abuse specialist, vocational specialist, psychiatrist/APN, peer support specialist, and a program assistant to provide these services in the community. The members of the team work together with the individuals served to promote symptom stability and appropriate use of medication; restore personal, community living and social skills; promote and maintain physical health; establish access to entitlements, housing, work and social opportunities; and promote and maintain the highest possible level of functioning in the community. This program is funded by, and is part of the system of public services offered by Delaware DSAMH.
