Case Manager II

Program Summary
Critical Time Intervention (CTI) locates and connects people to adequate, affordable housing units, and provide support as they transition from an institutional setting to a new residence. Linkages to mental health services, collaboration with community providers, and development of natural support systems are additional supports provided by CTI. During this critical time, CTI monitors and assesses these supports to ensure their effectiveness in maintaining an individual's new residence and breaking the cycle of homelessness.

Responsibilities/Duties

Duties and Responsibilities:

  1. Assess and evaluate new consumers for appropriateness for CTI and other services.
  2. Actively engage and support clients at CHOC with service connection and other identified needs:

-Assist assignedclients with obtaining mainstream benefits

-Complete budgets, housing goalplans, and service prioritization assessments

-Update budgets and housing goalsas situations change and/or goals are obtained


  1. Relate positively and work in collaboration with other CTI specialists in a team approach to share consumer information, insights, and suggestions.
  2. Establish and maintain empathetic, trauma informed, non-judgmental relationships with clients.
  3. Participate in case management meetings at area shelters, county homeless meetings, and all other appropriate meetings/training outside of the agency.
  4. Participate in weekly individual supervision or as required by Clinical Director/Supervisor.
  5. Participate in weekly group supervision or as required by Clinical Director/Supervisor.
  6. Follow all Your Way Home guidelines regarding data collection and data entry.
  7. Coordinate client appointments with direct service staff, external service providers, and/or public transportation so they can get to their appointments on time.
  8. Accompany consumers into the community, when needed.
  9. Gain access to services with aggressive and creative attempts to help consumers obtain resources that are needed. This includes a strong role as a consumer advocate.
  10. Monitor service delivery by maintaining regular contact with consumers and service providers/programs involved with the client.
  11. Create linkages to formal and informal supports to strengthen client’s ties within the community.
  12. Monitor consumers on a regular basis as determined by consumer’s level of need and note any changes that may impact his/her treatment/recovery.
  13. Maintain clear and concise clinical documentation of all client and provider contacts/referrals and audit consumer charts as required compliance with funder and licensing requirements. 
  14. Maintain weekly billing productivity requirements as established by CTI Directors.
  15. Other related duties as assigned.


Documentation Responsibilities:

  1. Review and submit time sheet by the payroll deadline.
  2. Complete required all documentation for data collection and billing requirements established by the licensing and funding agencies of CTI.
  3. Ensure that HMIS (Homeless Management Information System) case notes are recorded, entered properly and updated regularly.
  4. Ensure that discharge summaries are completed in a timely fashion.

Required Experience

  • Experience working with a homeless population and providing outreach to homeless individuals is highly desired. 

Required Education

  • A Bachelor's degree in social work, psychology, counseling, social work, education, or other social service related field with at least 2 years mental health/D&A experience ispreferred.

Required Qualifications


  • Valid PA Driver’s License required. 
  • Proficiency with Microsoft Office Suite
  • Experience working with Recovery Model and Trauma-Informed practices
  • Experience and knowledge using Motivational Interviewing and Stages of Change
  • Must be able to meet with clients in crisis from anywhere within Montgomery County within 1.25 hours of their initial contact when working on-call. 

The ability to interact professionally with clients, theirfamily members, and other service providers using a trauma-informed approach isessential. Must be able to work with a teamand demonstrate flexibility. Must bewilling to work out of a homeless shelter and travel into the community withclients for appointments and meetings. 

Program Summary

CTI is a trauma-informed, blended case managementservice that provides intensive support to adults experiencing homelessness inMontgomery County who also have significant mental health needs.

CTI’s primary initiative is to locate and connectpeople to adequate, affordable housing units, and provide support as a clienttransitions from a shelter or other setting to a new residence. CTI can supportclients with linkages to mental health and substance use services, collaboratewith community providers, and assist in the development of a client’s naturalsupport system. During this critical time, CTI monitors and assesses thesesupports to ensure their effectiveness in maintaining their new residence andbreaking the cycle of homelessness.
