Position Summary
  
Under the supervisions of the Program Director, COMPASS Registered Nurse provides the medical and psychiatric assessment and direct care as appropriate to state laws.  These services are designed for the consumers diagnosed with a serious and persistent mental illness and having met the criteria for participation in the COMPASS Mobile Medication Program.  These responsibilities include: Initial assessments prior to admission, completion of program admissions, initial nursing assessment of the medical and psychiatric status of the consumers, collaboration with the physician, instruction regarding disease process, medication education, medication compliance, symptom management and activities and interventions to enhance well being.  In addition, the Registered Nurse will assume an active role in the team process designed to maximize collaborative efforts.
 
The Registered Nurse, will be responsible to lead mobile, in-home or community based medication management services to individuals residing within Schuylkill County, difficulty managing prescribe medication regimens independently and/or consistently.
More specifically, this position will develop and implement individualized treatment plans  designed to develop and/or enhance the participants skills needed to effectively and independently manage their medication regimen, negotiate community resources, and provide interventions and psycho-educational tools that support skill development.
 
Essential Duties and Functions
  • Support and promote the mission statement and values of Resources for Human Development and the COMPASS Mobile Medication team.
  • Obtain the consumer medical history and assess for level of care.
  • In collaboration with the consumer and physician, to formulate a service plan designed to promote good health practices and participation in the recovery process.
  • Conducts the review of the service plan with particular focus on psychiatric and physical health stabilization/management.
  • Assists in obtaining and/or interpreting medical information to improve the team member’s knowledge base regarding consumers’ status, needs and strengths. Communicates with physicians to clarify/mediate.
  • Performs the nursing process (assessment, identification of problems, planning, implementation and evaluation).
  • Assess signs and symptoms and report pertinent information to the consumer’s FMD or psychiatrist.
  • Perform skilled nursing activities as prescribed by the consumer’s physician and within the purview of the law. Interprets and carries out physician orders accurately and promptly.
  • Initiates referrals to other disciplines and other agencies.
  • Consults with team leaders and Clinical Coordinator regarding high risk, problem prone consumers.
  • Participates in specialty trainings for team members and consumers. The RN will assure that all operating procedures and regulations are followed.
  • Must understanding and interested in working with persons who have severe and persistent mental health challenges.
  • Expected to engage individuals in a trusting, supportive and professional relationship, while providing emotional support within professional boundaries.
  • Meet established productivity requirements set by the program guidelines.
  • Interacting with individuals, families and non-agency personnel in a professional manner.
  • Knowledge of psychotropic medications and potential side-effects.
  • Knowledge of  prescribed psychotropic and/or medical medications, side-effects and times medications are scheduled to be administered for each assigned individual.
  • Completing reports or other written work assigned within the expected time frame.
  • Provide mobile, in-home and follow up services to assigned individuals.
  • Complete required and time-dependent documentation/data collection.
  • Maintain professional and timely treatment communication and documentation.
  • Operate in a professional manner consistent with RHD's organization values.
  • Concurrent telephonic continued stay reviews with Managed Care Organizations.
  • Following through on directives given by direct supervisor or Program Director.
Specific Job Duties
  • Complete initial and on-going monthly assessments including but not limited to: Nursing Assessments, Vital Signs, Mental Health Status Exams and Biopsychosocial Assessments.
  • Conduct initial and on-going monthly medication reviews including: updating medication information, review of current or potential side-effects and allergies and hypersensitivities.
  • Administer injectable medications to participants when needed.
  • Confirm participant’s medications and medication prescribers.
  • Scheduled and participate in Case Review meetings with program funders.
Qualifications
  • Current unrestricted Pennsylvania Registered Nurse.
  • Provide a copy of current Registered Nurse License.
  • Minimum 3 years of nursing experience with a minimum of 1 year psychiatric experiences servicing adults with serious and persistent behavioral health concerns.
  • Previous experience working with an Electronic Health Records database.
  • Valid Pennsylvania Driver’s License and proof of insurance.
  • Must have access to a personal registered and insured vehicle.
  • Ability to communicate professionally in both verbal and written forms.
  • Ability to work independently and manage multiple  expectations.
  • Proficient computer skills.
