*Job Duties/Responsibilities:
Crossroads Community Services is committed to a recovery focus with emphasis on a "team approach" engaging and supporting clients with serious and persistent mental illnesses residing in Carbon, Monroe, Pike and Schuylkill Counties to promote successful community tenure.
Job duties include (but not exclusively):
Assessments
Goal Planning
Linkage and Referrals to community resources
Gaining access to services
Monitoring of services delivery
Crisis Planning and problem resolution
Informal support network building
Following all operational procedures and regulations
Assisting clients in attending community appointments
Moving clients into their residences
On-call after hours/weekends on a rotation basis
 
*Job Requirements/Minimum Qualifications:
This is a Monday-Friday, 8am-4:30pm position, with on-call responsibilities during nights, weekends, and holidays on a rotating basis.

This position requires a Bachelor’s degree in a human service field. Bi-lingual and previous experience with case management and/or Mental Health/Drug and Alcohol desired. 
The ideal candidate manages multiple priorities, is flexible, and has excellent communication skills.  Must be able to bend, stand, and sit, for long periods of time and lift up to 25lbs. Must possess a valid driver’s license, car insurance and a registered/inspected vehicle and live with a 30 minute radius (approximate) due to on call responsibilities.
