Description/Job Summary

RHD is now offering a $500 Sign on Bonus to be paid upon successful completion of 90 days employment!

**Street Outreach takes place outside in the community and surrounding woods where clients/those experiencing homelessness may be temporarily residing. Applicant must be willing and able to work outside in all weather climates, climb/hike to tent locations, and be able to carry supplies such as water and backpacks of supplies to program participants. Working hours can be at times non-traditional as community meals take place later in the afternoon/early evening or on weekends.**

Case Management

The Case Management staff meet each client where they are at, assessing both strengths and needs. Together they develop a goal plan to reach the client’s greatest level of self-sufficiency. Case Managers help clients access health and mental health services; entitlement  programs such as Food Stamps, TANF, SSI/SSD; other needed Community Resources, Coordinate with family members and other supports in client’s life to develop wrap around services to build a strong foundation; Link clients to employment services and housing opportunities; Support clients for up to six months.   


  1. Engage in a two - three (2-3) week training period with various community providers to learn about the resources offered in the community.
  2. To support and promote the mission statement and values of Resources for Human Development, Inc.
  3. To engage people in a trusting and supportive relationship, providing emotional support and focusing on target goals.
  4. To collaborate with individuals and others in the completion and review of a strengths based assessment/service plan.
  5. To assist the person to develop an individualized, self-sufficiency service plan based on the specific needs of the individual.
  6. Assist in the maintenance and distribution of a current referral/pocket resource guide for clients
  7. Maintain relationships with the various programs and non-traditional resources in our community to promote our service.
  8. Engaging clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries.

9.    Maintain required records accurately, comprehensively and in a timely manner.

  1. Keeps required statistics and submits reports as requested; entering data into the HMIS system
  2. Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, and compliance with all regulatory requirements.
  3. Provide street outreach to the homeless to include intake/assessment and case management where the client is staying, the setup of tents/temporary shelters, providing hygiene products and other necessities, and linkage to services 
  4. Provides liaison with family members, community agencies, advocacy persons/groups and hospitals when appropriate.
  5. To work in a collaborative and collegial manner with other staff both intra- and inter-agency in assisting both the program’s clients and the program to meet identified goals.
  6. Insures the confidentiality of all client and/or employee-related information.

16   To maintain one’s own physical, mental and emotional well-being to maximize the healthy functioning of the program.

  1. Flexibility in location and hours of service provision
  2. Attend meetings as needed to include Community Meals on a weekly basis
  3. Report in writing any conflict of interest of any individuals you may know personally
  4. Attend weekly supervisions with Supervisor, Team meetings, and Staff meetings.
  5. Represent RHD and Crossroads/Street 2 Feet at Community Events, speaking engagements, and other outreach opportunities to promote awareness of the program and the needs of the clientele.

Performs tasks which are supportive in nature to the essential functions of the job, but which may be altered or re-designed depending upon individual circumstances.

Required Experience

Familiarity with Monroe County geography and resources preferred.

Required Education

Bachelor's Degree in a Human Services field preferred

Reliable Transportation

Must be able to pass a Criminal background check and PA Child Abuse Clearance

Program Summary

Street 2 Feet Outreach Center is a program under RHD Crossroads Community Services. Street 2 Feet is the only daytime resource center for those experiencing homelessness in Monroe County PA. All staff are cross trained in all of our services provided at Street 2 Feet such as Case Management, Coordinated Entry Intakes, and participating in our client employment program, Pocono 3C.
