The Case Management staff meet each client where they are at, assessing both strengths and needs. Together they develop a goal plan to reach the client’s greatest level of self-sufficiency. Case Managers help clients access health and mental health services; entitlement programs such as Food Stamps, TANF, SSI/SSD; other needed Community Resources, Coordinate with family members and other supports in client’s life to develop wrap around services to build a strong foundation; Link clients to employment services and housing opportunities; Support clients for up to six months.
Essential Functions of the Position:
To support and promote the mission statement and values of Street 2 Feet and RHD, Inc.
To engage people in a trusting and supportive relationship, providing emotional support and focusing on target goals.
To collaborate with individuals and others in the completion and review of a strengths based assessment
To assist the person to develop an individualized, self-sufficiency service plan based on the specific needs of the individual.
Maintain a current referral list for clients.
Maintain relationships with the various programs in our community to promote our service.
Engaging clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries.
8. Maintain required records accurately, comprehensively and in a timely manner, including data entry into the Homeless Management Information System.
Keeps required statistics and submits reports as requested.
Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, and compliance with all regulatory requirements.
Provides liaison with family members, community agencies, advocacy persons/groups and hospitals when appropriate.
To work in a collaborative and collegial manner with other staff both intra- and inter-agency in assisting both the program’s clients and the program to meet identified goals.
Insures the confidentiality of all client and/or employee-related information.
14. To maintain one’s own physical, mental and emotional well-being to maximize the healthy functioning of the program.
Attend Monroe County Homeless Advisory Board Meetings and community meetings/ events.
Participate in the development and implementation of program fundraising events and activities.
Participate in public speaking events to bring program awareness to the community.
Attend program fundraisers and community events.
Represent RHD and the program out in the community with professional demeanor.
Maintain a valid PA Driver License and active car insurance.
Report in writing any conflict of interest in working with clients of the Day Center that you may know personally
Participate in weekly supervisions with Supervisor to discuss client concerns/needs.
Approximately two years experience in the social services field preferred
Must have a Bachelor degree in Human Service field (preferably in social work); Certification in Drug & Alcohol a plus.
Valid Drivers License and active vehicle insurance
Must be able to pass a background Check and Child Abuse Clearance
This position works out of our homeless day center, Street 2 Feet Outreach Center located in Stroudsburg. We are the only daytime resource center for those experiencing homelessness in Monroe County. The ideal candidate will also be assisting at times with Street Outreach, Coordianted Entry Assessments, and our Pocono 3C program (garbage pick up employment program for clients). Schedule will also include rotating Saturdays.
