The role of the Coordinated Entry Specialist is to complete VI-SPDAT assessments on all persons presenting to the Coordinated Entry Access Site (Street 2 Feet) as homeless, entering them into the Community Queue for housing services/resources and/or providing Diversion services to prevent from becoming homeless as needed. Responsible for collaborating with the PA 211 Call Center and other Regional Coordinated Entry Access Site staff for peer learning and problem solving. The Coordinated Entry Specialist will follow up with households on the Coordinated Entry Queue and provide status updates. When not doing these tasks, the person in this role will provide support to the Street 2 Feet staff and assist with programmatic and consumer needs.
Essential Duties and Functions
1. To support and promote the mission statement and values of Street 2 Feet and RHD, Inc.
2. To engage people in a trusting and supportive relationship, providing emotional support and focusing on target goals.
3. Maintain a current referral list for clients.
4. Maintain relationships with the various programs in our community to promote our service.
5. Engaging clients in a trusting and supportive treatment partnership, providing emotional support while maintaining professional boundaries.
6. Maintain required records accurately, comprehensively and in a timely manner, including data entry into the Homeless Management Information System.
7. Keeps required statistics and submits reports as requested.
8. Cooperates with, participates in and supports the adherence to all internal policies, procedures and practices, and compliance with all regulatory requirements.
9. Insures the confidentiality of all client and/or employee-related information.
10. To maintain one’s own physical, mental and emotional well-being to maximize the healthy functioning of the program.
11. Represent RHD and the program out in the community with professional demeanor.
12. Maintain a valid PA Driver License and active car insurance.
13. Report in writing any conflict of interest in working with clients of the Day Center that you may know personally
14. Participate in weekly supervisions with Supervisor to discuss client concerns/needs.
15. Create and/or update household records in the CoC’s ClientTrack Homeless Management
Information System (HMIS).
16. Field and record the answers to a brief set of pre-screening questions in HMIS to determine
the household’s eligibility for entering the housing crisis response system.
17. Attempt to divert any household seeking to enter emergency shelter through a strengths based
conversation about creative solutions to avoid the trauma (to the household) and
expense (to the system) of entering emergency shelter, thereby preserving limited shelter
beds to people who have no other safe place to stay.
18. For households that are eligible for CoC housing crisis response system services, the
a) field the Vulnerability Index – Service Prioritization Decision Assistance
Tool (VI-SPDAT) assessment tool (different versions for families, single adults and
Transitional Age Youth) and enter the head of household’s answers directly into HMIS;
b) provide a direct referral to emergency services (such as outreach or shelter) if needed after attempting Diversion; and/or
c) place the household on a Community Queue for housing services (such as Rapid Re-Housing or Permanent Supportive Housing).
19. Provide Community Queue status updates to households waiting for referrals to services.
20. Comply with all CoC Coordinated Entry System policies and procedures.
21. Participate in annual trainings on the use of HMIS, VI-SPDAT and Diversion strategies.
22. Participate in quarterly meetings (in-person, webinar or telephone as appropriate) with
other regional Coordinated Entry Access Site providers for peer learning and problem solving.
23. Provide ongoing updates and feedback to Director regarding Coordinated Entry System.
Bachelor’s degree in a Human Service field
Must be able to pass a Criminal background check and PA Child Abuse Clearance
This position will work out of our homeless day center, Street 2 Feet Outreach Center. We are the only homeless daytime resourcec center in the area and function as the walk in Access Site for the Coordianted Entry 211 system, the streamlined service for housing assistance. In addition to Coordinated Entry, the ideal candidate will also be assisting with Street Outreach, Case Management, and the Pocono 3C (Garbage Pick Up employment program for clients). Schedule may also include rotating Saturday hours.
