Description/Job Summary

RHD's COSP is seeking a full-time Care Coordinator professional to work in the Philadelphia area. 



RHD's Children's Services provides intensive community-based services to emotionally or developmentally challenged children and adolescents (from birth to age 21). Service needs and treatment recommendations are based on team decisions to ensure that all clinical, educational, and social issues are identified.

Responsibilities/Duties

The Care Coordinator will:

Coordinate intake and scheduling of psychological evaluations (include parents, BSC’s, DHS, other agencies involved the child’s system of care)
Ensure timely coordination of requests for ongoing authorization of BHRS services
Compile managed care pre-authorization documents
Case Management liaison to CBH for child’s services and team collaboration 
Support families in resource coordination for Medical Assistance and recommendations made by evaluators
Function as liaison to school and community based agency partners

Required Education

BA/BS in a related social services field of study

Required Qualifications

Must have a valid driver's license and own vehicle

Candidates are required to have updated Child Abuse, PA Criminal, and FBI clearances within 30 days of hire
